Overview
Respiratory infections are inflammatory illnesses of the airways and lung parenchyma caused by viruses, bacteria, fungi and other pathogens, conventionally divided into upper-tract infections involving the nose, sinuses, pharynx and larynx and lower-tract infections involving the bronchi and alveoli. Transmission is chiefly by respiratory droplets, aerosols and contact, and severity ranges from self-limiting coryzal illness to pneumonia, respiratory failure and death, with disproportionate impact on infants, older adults and the immunocompromised. Diagnosis combines clinical assessment with microbiological, serological and imaging studies, while management spans supportive care, antimicrobials directed by aetiology, vaccination and stewardship to limit resistance. Work in this area is shaped substantially by COVID-19, including narrative review of SARS-CoV-2, severity and risk in older adults, and the One Health dimension of animal involvement, alongside influenza-related vitamin D status, diaphragmatic pacing as an alternative to mechanical ventilation, mortality profiling in a pneumophthisiology department, antibiotic prescribing for upper respiratory tract infection, and review of cough management. Environmental determinants such as climate change and air quality, and host factors including nutritional and vitamin D status, are also represented. The journal publishes peer-reviewed clinical, microbiological and public-health research on respiratory infections, their prevention and treatment.
