Overview

The larynx is the cartilaginous organ of the neck situated at the upper end of the trachea, where it serves three principal functions: producing voice, protecting the lower airway during swallowing, and regulating airflow during breathing. It is built from cartilages, including the thyroid, cricoid, and paired arytenoids, joined by muscles and ligaments, and houses the vocal folds, whose tension and position are controlled by intrinsic laryngeal muscles innervated by branches of the vagus nerve. Phonation occurs as exhaled air vibrates the adducted vocal folds, while during swallowing the epiglottis and laryngeal closure divert food away from the trachea. The larynx is a frequent site of disease, most importantly squamous-cell carcinoma, which is strongly linked to tobacco and alcohol use and may require partial or total laryngectomy with attendant effects on voice and swallowing. Other conditions include benign lesions, inflammatory disorders, and structural abnormalities. Assessment and management draw on endoscopy, imaging, prognostic indicators such as the neutrophil-to-lymphocyte ratio, and surgical techniques that aim to balance oncological control with preservation of function. Tissue-engineering and regenerative approaches are explored to reconstruct laryngeal structures. Research in this area addresses laryngeal cancer, the functional outcomes of conservation and total surgery, perioperative risk, and tissue-engineering strategies for laryngeal repair.
