Reduced Tissue Oxygenation and Altered Valsalva Hemodynamics in Young Adults with Type 1 Diabetes

Aims Cardiac autonomic neuropathy is currently an untreatable progressive complication of type 1 diabetes (T1D). Impaired microcirculation is a suspected cause of nerve degeneration in TID. We investigated whether cardiovascular autonomic reflexes often used as indices of nerve functions, are associated with indices of microcirculatory function in young adults with T1D compared with non-diabetic controls. Methods In a cross-sectional study, 15 adults with T1D and 15 age-matched controls (20-40 years) underwent standardized cardiovascular autonomic reflex tests. Continuous recordings of electrocardiogram, cardiac vagal tone, beat-to-beat blood pressure and transcutaneous tissue oxygen (tcpO₂) and carbon dioxide partial pressures (tcpCO2) were done. Results Despite preserved baroreflex, parasympathetic, and sympathetic functions assessed using cardiovascular reflex tests, the individuals with T1D exhibited reduced baseline tcpO2 compared to the controls (37.5±3.75 vs. 49.6 mmHg). During the Valsalva manoeuvre, individuals with T1D exhibited a reduced systolic blood pressure response in phase I (31±10 vs. 43±18 mmHg) and early phase II (-1±15 vs. -18±17 mmHg), and an increased systolic (31±15 vs. 18±14 mmHg) and diastolic (45±11 vs. 33±16 mmHg) response in late phase II compared to controls. The early phase II diastolic response was inversely associated with baseline tcpO2. Conclusion The altered hemodynamic response to the Valsalva manoeuvre is suggestive of possible reduced arterial elasticity, higher vascular resistance, and splanchnic sympatho-vagal imbalance in T1D despite normal autonomic reflex ratios. The concomitant evidence of reduced tissue oxygenation and altered hemodynamics may represent early signs of dysautonomia but require longitudinal validation.  

Anchors of the Self: Cultivating Life Longings (Sehnsucht) and Goals in LGBTQ+ Therapeutic Discourse

We have highlighted the significant harm caused by internalized homophobia in LGBTQ+ individuals and the dimensions of internalized identity that often dim and redirect Motivations and Longings for a meaningful and valuable life for individuals in the LGBTQ+ community. We surveyed the dimensions of Life Goals and Longings as a basis for building motivation and a sense of worth in the individual, which forms the foundation of emotional well-being. From the perspective of the need for Longings and Life Goals, we identified relevant focal points in empowering work to establish these anchors within LGBTQ+ individuals. This process primarily focuses, alongside interpersonal and social dimensions, on the internal feelings associated with an individual's sense of identity. We emphasized the importance of having unique, personally tailored Longings for LGBTQ+ clients that are rooted in the dimensions ('virtues') of what is called 'character.’ As Hammack and Toolis (2014) articulated, we did not focus on others' perceptions of the LGBTQ+ individual as the sole central factor in constructing an individual's sense of self-esteem, although it is an important factor. Instead, we focused on the view that LGBTQ+ individuals mainly construct their self-esteem based on their perception of their values relative to those of society around them. An individual's values and Goals are at the center of constructing their sense of self-esteem when compared to society’s ‘correct' Goals. Individuals see themselves in character traits, less so in terms of function, in relation to their environment, and thus define their self-esteem as being 'good' or 'bad' in their own eyes 666467.

Prevalence of Burnout, Secondary Traumatic Stress, Anxiety, and Depression Among Maternal and Neonatal Healthcare Staff in Two Tertiary Hospitals in Zambia

Background Healthcare worker stress, anxiety, burnout, and trauma have been widely documented across global healthcare systems. Staff in maternal and neonatal units frequently encounter emotionally distressing events and work under high pressure with limited resources. In Zambia, despite some gains in reducing maternal and neonatal mortality, fatality rates remain above national targets, exposing staff to repeated occupational trauma. Objective(s) This study aimed to assess the psychological well-being of staff in maternal and neonatal intensive care units, identify specific needs and stressors, and develop practical recommendations to improve resilience and support staff mental health. Methods A mixed-methods cross-sectional study was conducted at Ndola Teaching Hospital (NTH) and Arthur Davison Children's Hospital (ADCH), two major tertiary hospitals in Zambia. Data were collected over four days in February 2025. The study included 87 (out of 161) healthcare professionals, including nurses, midwives, and doctors. The Goldberg Anxiety and Depression Scale (GADS) and the Professional Quality of Life Scale Version 5 (ProQOL 5) were used to collect quantitative data, which were analysed using descriptive statistics and 95% confidence intervals (CI). Open-ended survey questions provided qualitative data, which were analysed using thematic analysis. Ethical approval was granted by the hospital ethics committee. Results High rates of clinical symptoms were observed. The overall prevalence of symptoms in the past 30 days was: anxiety (62%), depression (68%), burnout (50%), and secondary traumatic stress (46%). Junior doctors demonstrated the highest rates of depression (83%) and burnout (67%). The obstetrics and gynaecology and labour wards had the highest rates of secondary traumatic stress symptoms. Key qualitative themes identified were professional stressors, team and leadership issues, and the emotional burden of the work. Conclusion This study highlights an urgent need for both psychological and organizational support for maternal and neonatal healthcare staff in Zambia. The findings indicate that burnout and secondary traumatic stress are highly prevalent, particularly among junior doctors and those in high-risk wards. Targeted interventions at both systemic and individual levels are necessary to protect the well-being of healthcare workers and improve patient outcomes.

Research Study Reveals Factors That Impacted Ohio Funeral Director Attrition and Retention Rates

A phenomenological qualitative study was conducted to obtain facts and details about the lived experiences of Ohio funeral directors during their first five years of licensure. The goal of the study was to understand these lived experiences as told by funeral directors to understand better the factors that impact the attrition and retention of new licensees. The data was analyzed using the Colaizzi 1978 method. The collected data aided in professional development programs offered to Ohio funeral directors and embalmers which increased the discussion and interest in factors that impact attrition and retention rates of new funeral director licensees at the local and national levels. The information from the study can be applied to allied helping professions such as healthcare, ministerial, and education. The stories and experiences as told by Ohio funeral directors provides new insight into the factors that impact attrition and retention rates of new licensees.

Peripheral Third Cranial Nerve Palsy in A Patient With Pediatric Form of Multiple Sclerosis

Multiple Sclerosis has traditionally been considered an inflammatory and autoimmune disease of the central nervous system. However, peripheral cranial nerve involvement has been described previously in eight cases, raising the hypothesis of a disease spectrum between central and peripheral nervous system. We hereby present a case of a 12 years old girl diagnosed with Multiple Sclerosis who presents with complete unilateral third cranial nerve palsy. Complete clinical, laboratory and radiological work-up was consistent with demyelinating disease. We conclude that demyelination in Multiple Sclerosis can affect in some cases both the central and peripheral nervous system.

The Beneficial Effect of Two Nutraceuticals in Flea Allergy Dermatitis Itch Control: A Comparative Study

This prospective randomized double-blinded multicentric study aimed to assess the efficacy of a new nutraceutical in controlling itch and skin lesions caused by flea allergy dermatitis (FAD) and compare it with another oral product of proven efficacy. Forty-three dogs, of different age, breed and sex, with FAD were included and divided into two groups: 24 received product A and 19 product B. Both groups received the same antiparasitic treatment. A modified canine atopic lesion index (mCADLI) and pruritus visual analogic scale (pVAS) were recorded at days 0, 30 and 60. pVAS was evaluated both by the examining veterinarian (vpVAS) and the owner (opVAS). Results obtained were compared between the two groups at each time point. In both groups of dogs, a significant decrease of mCADLI, vpVAS and opVAS at day 30 and 60, compared to day 0, was observed. At baseline, the mCADLI scores of Group B were significantly higher than Group A (95% CI: -4.0 to 0.0, p < 0.05), with no differences at D30 and D60 (not significant, p > 0.05). In Group B, significantly lower values of vpVAS were observed at D30 compared to Group A (95% CI: -2.5 to 0.0, p < 0.05), but not at D60 (not significant, p > 0.05). The median values of pVAS in Group B were lower compared to Group A at both D30 (95% CI: -2.5 to 0.4, p < 0.01) and D60 (95% CI: -3.0 to 0.3, p < 0.05). Both nutraceuticals can be useful to control discomfort, skin lesions and pruritus due to flea allergy dermatitis and can be a valuable replacement for antipruritic drugs antihistamines, steroids or Il-31 inhibitors.

What Can a Stylometric Analysis Contribute to the Diary of Saint Faustina Kowalska?

Saint Maria Faustina Kowalska's Diary records the words of Christ revealed to her between 1934 and 1938. The frequency of the words used by God and Sor Faustina could be statistically determined by stylometric analysis since each has its own linguistic footprint. To do this, we analyze various parts of the Diary in three different editions (Polish, Spanish, and English). The results show a linguistic style characteristic of Sister Faustina and another to God. So this paper confirms that there was a conversation between the two.

RET 898-901Del mutant, a variant of unknown significance, has a durable response to Pralsetinib in a Medullary Thyroid Carcinoma patient

Background Patients with distant metastatic Medullary Thyroid Carcinoma (MTC) have an estimated 40% ten-year survival rate. Gain of function mutations in the REarranged during Transfection or RET gene in MTC can result in an aggressive phenotype resistant to traditional therapy. In this case report, we describe the treatment of an MTC patient with a unique RET kinase deletion mutation. Case presentation Since diagnosis, 21 years ago, this patient has had chronically elevated calcitonin levels (>40,000 pg/mL) that was unable to be controlled by conventual therapy and clinical trials. As result of uncontrolled MTC, metastatic disease was found in the spine, liver, and lungs. Circulating tumor DNA (ctDNA) analysis identified a RET 898-901Del mutation, reported as a variant of unknown significance. The treating physician identified that the deletion was in the activation loop of RET kinase and considered that the mutation was constitutively activating RET kinase. The patient was prescribed Pralsetinib, a small molecule inhibitor targeting the ATP binding site of RET. Pralsetinib treatment achieved a durable response and was able to significantly decrease serum calcitonin levels (<200 pg/mL) and tumor size. Conclusion This RET deletion mutation is a pathogenic mutation with comparable enzymatic activity to the more common RET M918T mutation. The case report highlights the versatility of structural biologic approaches to guide therapeutic decisions.

Knowledge And Preventive Practice Towards Covid-19 Infection Among Pregnant Women In Public Hospitals Addis Ababa, Ethiopia, 2022

Background The novel coronavirus COVID-19 has created massive challenges to public health worldwide. Pregnant women are an immune-depressed status which makes pregnant women generally more vulnerable to COVID-19 infection and severe illness. The present preventive measure practices have gaps. Therefore, the current study aimed to present accurate and latest information regarding preventive measures for COVID-19 infection among pregnant women. Methods and materials Institution-based cross-sectional study design was conducted on 422 pregnant women in public hospitals in Addis Ababa using a pretested structured questionnaire. Face-to-face interview on pretested and structured questions was conducted to collect the data between January 12 and February 15, 2022. The collected data were entered into Epi data version 4.4.2.2 and exported to SPSS window version 25 for analysis. Descriptive statics and multivariable logistic regression were analyzed. Odds ratio with a 95% confidence interval and p-value ≤ 0.05 were declared statistical significance independent variables. Result A total of 418 pregnant women participated, and the response rate was 99.05%. About 417 (99.8%) pregnant women reported never hearing about COVID-19. Of those who heard about COVID-19, only 49% and 54.3% had good knowledge and had good practice towards COVID-19 infection respectively. Age (26-30yrs (AOR=0.46, 95%CI: 0.014,0.12), no education (AOR=0.23, 95%CI: 0.099-0.52), Primary school (AOR=0.199, 95%CI: 0.104-0.4) and Secondary school (AOR=0.282, 95%CI: 0.14-0.55), divorced (AOR=0.15, 95%CI: 0.065-0.34) and widowed (AOR=0.16, 95%CI: 0.024-1.03)) were factors that associated with knowledge towards COVID-19 infection. Age (30-35yrs (AOR=0.334, 95%: 0.115-0.97) and >36yrs (AOR=0.28, 95%CI: 0.11-0.69)), no education (AOR=0.06, 95%CI: 0.019-0.18), being a civil servant (AOR= 0.28, 95%CI: 0.122-0.66), divorced (AOR=0.042, 95%CI: 0.01-0.18), having >4 family size (AOR=0.334, 95%CI: 0.169-0.66), no previous complication of pregnancy outcomes (AOR=0.019, 95%CI: 0.01-0.061), chronic health problem (AOR=14.66, 95%CI: 0.457-39.4) and two ANC visit (AOR=5.704, 95%CI: 2.41-13.5) were factors that associated with the practice towards COVID-19. Conclusion In this study area, only half of pregnant women had good knowledge and good practice about covid-19 infection prevention measures.  

Comprehensive Study of Structural, Optical and EPR Properties of Mn2+ Doped CdSe Nanocrystals

The synthesis of II-VI semiconductor nanocrystals doped with transition metal ions is particularly difficult. In the case of manganese doped CdSe nanocrystals produced via chemical route it is necessary to achieve small incorporation of manganese ions. The incorporation of manganese ions in CdSe nanocrystals has been confirmed by the study of Energy Dispersive Spectroscopy, Photoluminescence and Electron Paramagnetic Resonance spectra. The CdSe nanocrystals have cubic structure, the average crystallite size lies between 3-6 nm and increases with the variation of doping concentration of Mn2+ ions as characterized by X-ray diffraction. The structural properties of CdSe semiconductor nanocrystals are also studied by High Resolution Transmission Electron Microscopy, Scanning Electron Microscopy and UV- Visible Absorption Spectroscopy. The energy band gaps have been determined by UV/V is absorption study with the help of Tauc plot and are 2.54 eV, 2.27 eV and 2.24 eV as concentration of Mn2+ions in CdSe semiconductor nanocrystals increases from 0% to 0.75%. The crystallite size is also obtained from the UV-Visible absorption study; this varies from 1.69 to 1.78 nm with increase of Mn2+ concentration from 0% to 0.75% which is in agreement with X-ray diffraction result. Further, the magnetic and electronic properties of Mn2+ doped CdSe nanocrystals are studied using EPR spectra and the values of g factor, second-rank axial zero-field splitting parameter D and hyperfine parameter A are calculated. EPR measurements reveal that the dopant ions are incorporated in CdSe semiconductor nanocrystals.

Monk Fruit is a Safe, Natural, Non-Caloric Sweetener in Individuals with Type 2 Diabetes

Non-nutritive sweeteners are used as adjuncts in treating patients with diabetes to reduce carbohydrate intake. Monk fruit is an FDA-approved1 natural NNS2-. There is no information regarding the use of monk fruit in individuals with T2DM3. The primary objective was to compare the glycemic response to sucrose and monk fruit in individuals with and without T2DM. Twenty-six individuals with T2DM and 29 without DM4 underwent two oral glucose tolerance tests5 after a 250-ml sucrose- or monk fruit-sweetened beverage. Glucose levels were measured at 0, 30, 60, 90, and 120 minutes. An appetite scale was used before and after the procedure. Insulin levels were measured in ten randomly selected individuals from the control group. The glucose levels at 30 and 120 min were significantly lower in both groups in the monk fruit-OGTT. There was a peak response in the T2DM group at 90 min (median, IQR, 247 mg/dL, 203-293) with the sucrose OGTT, while no peak response was seen in the monk fruit OGTT. There were no differences between sweeteners in terms of appetite evaluation. The postprandial effect on insulin levels was flat with the monk fruit-sweetened beverage. In contrast, there was a significantly higher spike with the sucrose-sweetened beverage. In conclusion, monk fruit does not increase glucose levels in individuals with T2DM; thus, it could be a safe alternative as a substitute food sweetener.

Distribution and Conservation Challenges of Diurnal Large Mammals in Bayo Community Managed Forest, Salamago Woreda, South Omo Zone, Southern Ethiopia

An ecological study on diurnal mammals was carried out in Bayo Community Managed Forest located in Salamago Woreda, South Omo Zone. The objective of the study was to investigate the distribution and conservation challenges of diurnal large mammals in the study area. Based on the habitat type and topography of the study area, total of 11 transect, i.e 7 in forestland, and 4 in Wooded Grassland were laid to collect the data. Besides direct methods, indirect methods such as faecal droppings, fresh tracks, carcass or shell count, den (burrow), hair, and digging were used. Questionnaire and focus group discussions were also used to assess anthropogenic threats in the study area. Data were analyzed using descriptive statistics, SPSS and QGIS software. A total of 20 species of diurnal large mammals belonging to six orders and eight families were identified. The species identified were Cercopithecus pygerythrus, Cercopithecus aethiops, Papio anubis, Erytherocebus patas, Cercopitheus neglectus, Colobus guereza, Equus quagga, Traglaphus strepsiceros, Traglaphus imberbis, Traglaphus scriptus, Medagua guentheri, Sylvicapra grimma, Kobus ellipsiprymnus defessa, Syncerrus caffer, Potamochoerus larvatus, Phacochoerus africanus, Hylochoerus meinertzhageni, Hystrix cristata, Orycteropus afer, and Phataginus temminckii smutus. Seasonal variation in the between habitat types (χ2 = 4.849, df= 1, p<0.05). Totally, 685 and 600 mammals were counted during wet and dry seasons, respectively. On habitat basis, 683 and 602 animals were recorded in forestland and wooded grassland habitats, respectively. Major threats in the study area include poaching, fire, grazing, fuelwood extraction, population growth, habitat modification, overharvesting of resources and invasive species. About 98.44% of respondents had a positive attitude towards Bayo Community Managed Forest. The interference of local community has had the impact on mammal’s species. Habitat based mammals’ management involving participation of Woreda and Zonal Government is recommended for sustainable. The local government should promote the study area and provide appropriate support for its conservation.

Outcome of traumatic brain injury and its associated factors among pediatrics patients treated in Amhara national regional state comprehensive specialized hospitals, Ethiopia 2022.

Background Traumatic brain injury in pediatrics is one of the commonest causes of morbidity, disability and mortality worldwide. In low- and middle-income countries Study showed that death of pediatrics from traumatic brain injury was 7.3%. However, there is limited data towards the outcome of traumatic brain injury and its associated factors in Ethiopia. Objective To assess the outcome of traumatic brain injury and associated factors among pediatrics patients in Amhara National Regional State Comprehensive Specialized Hospitals, Ethiopia. Methods An institution based retrospective cross-sectional study was conducted among 423 pediatrics patients from January 1, 2019 to December 30, 2021, and data extraction period was from May 16 to June15, 2022. Systematic random sampling technique was employed to select the study participants. Data were collected from patient charts and registry books by using a data extraction tool. Data were entered into the Epi-info version 7 and analysis was done by SPSS Version 25. Both Bi-variable and multi-variable analyses were employed to identify factors associated with outcome of traumatic brain injury. Result From 423 sampled study participant charts 404 of them had complete information with response rate of 95.5% and included in the final analysis. The overall unfavorable outcome of traumatic brain injury at discharge was found that 12.13% (95% CI: 9.1% - 15.7 %). Sever traumatic brain injury (AOR: 5.11(CI :1.8-14.48), moderate traumatic brain injury (AOR:2.44(CI:1.07-5.58), Hyperglycemia (AOR: 3.01 (CI:1.1-8.04), sign of increased intracranial pressure (AOR:7.4(CI:3.5-15.26), and medical comorbidity (AOR: 2.65(CI:1.19-5.91) were predicted of unfavorable outcome of traumatic brain injury pediatrics patient. Conclusion and recommendations twelve present of traumatic brain injury results unfavorable outcome. Sever and moderate form of traumatic brain injury, hyperglycemia, signs of increased intracranial pressure, and medical comorbidity were factors associated with unfavorable outcome of traumatic brain injury in children. Therefore, it is preferable to improve accesses to acute and post-acute care services to lower the unfavorable outcome of traumatic brain injury in children.

Distribution of protected area and large-sized mammals order in Ethiopia

Ethiopia is one of the countries found in the eastern horn of Africa, which is endowed with beautiful landscape and topography from which diverse habitats and fauna composition. The objective of this paper is to review relevant documents on distribution of protected area and large-sized mammals order in Ethiopia. The Method of the data were by compiling related information from different sources like published articles grey literature and other office reports from concerned institutions. Altitudinal and geographical variations made the Ethiopia to be among the biodiversity-rich nations in the world. Mammal fauna in Ethiopia consists of 326 species, among 144 genera, 43 families and 14 orders; out of which 32 mammals of the country categories under critically threated group. Among the mammals in Ethiopia eight of them are endemic. Among mammals’ antelopes are flagships (charismatic). The large mammals are mainly concentrated in the south and southwest border.

Knowledge, Attitudes and Practices towards Infection Prevention Control among Healthcare Workers in Selected Hospitals Located in Karongi district, Rwanda

Most hospitals have infection prevention and control committees but some of them are struggling to maintain and to apply infection prevention and control protocols due to lack of enough resources. Healthcare workers’ (HCWs) knowledge, attitude and practices (KAP) of infection prevention and control procedures are crucial for effective infection prevention and control (IPC). The study aimed to assess HCW’s KAP towards IPC in Rwanda. A cross-sectional hospital based study was directed in three hospitals in Karongi district from February to March 2022. Data were collected from 215 healthcare workers using a pre-tested self-administered questionnaire using a stratified sampling technique. Data were collected, checked, coded, and entered into the Kobo Collect Toolbox before being transferred to SPSS version 21 for analysis. Bivariate and multiple logistic regression analyses were performed. The KAP Score was also calculated. P-values of less than 0.05 were considered statistically significant. We found that 50.7% of HCWs were males, 63.3% were between the ages of 18 and 58.2% had a secondary education. The overall 78.6% of HCWs demonstrated high level of knowledge, 79.5% with a positive attitude, and 63.3% with good IPC practice. The results revealed that being over 45 years old(AOR=3.1;95%,CI=(2.16-5.25; p=0.024) having university level(AOR=3.3); 95%CI=(1.56-7.56;p=0.035), working experience between 5-10years(AOR=1.7; 95%CI=(1.37-5.45); (p=0.003), having high level of knowledge (AOR=2.7;95%CI: (1.68–7.95; p=0.045)and positive attitude(AOR=2.3; 95%CI:(1.36-7.72); p=0.017) towards IPC were associated with IPC good practice. Improving institutional supplies such as hand hygiene supplies, PPE, water supply and other facilities can improve safe infection prevention and control.

Emerging Demands of Nutraceuticals (Functional Foods) Among the Women During Pandemic: An Intensive Exploratory Study

Corona pandemic resulted in huge burden on health care service sectors. Although pharmaceutical is the one of the largest industry in the world and predominant in health care services yet sudden outbreak of disease has questioned our ability of fighting back. The people across the globe have become more aware regarding health and wellness, hence they looking for other natural, reliable, affordable and available options. The nutraceuticals or functional foods are expected to have various health benefits and people are looking for more advance nutraceuticals for better immunity. An intensive exploratory study was conducted in one of the most reputed Banerjee’s Clinic located at Khamtarai region, Raipur, Chhattisgarh where 237 female participated in the questionnaire survey. The study result revealed that majority of women (52%) age 34-41 years use neutraceuticals for better health and to boost their immunity. Furthermore, the females who are working both public and private sectors and are financially independent are aware of health benefits of nutraceuticals/functional foods and spend on nutraceuticals for overall wellness. Moreover, the women preferred tablets form of nutraceuticals rather than powder or liquid form as mode of consumption.The study further shows that women who are professionals prefer to take daily functional foods or nutraceuticals for good health, immunity and beauty.

Fresh Water Snails of Public Health Importance in Canals in Okigwe Imo State Nigeria: Their Infectivity and Implications for Control

Studies have linked freshwater snails with human flukes as a result of repeated contacts with contaminated water. This study is aimed to determine the distribution of freshwater snails and their infectivity rates along human dwellings in Okigwe Imo state using Physico-Chemical parameters. Freshwater snails were collected by using long plastic spoons and with hand-picking method. The Physico-Chemical parameters were determined with appropriate instrument and freshwater snails harvested from the site were morphologically identified. One thousand four hundred and nine (1409) freshwater snails belonging to the species Lymneae and Bulinus were collected once a week throughout the period of study with the relative percentage of 43.9%; 29.9% ;26.2% respectively. The infectivity rate was highest in snails that were collected in the month of May compared to those collected in June and July, the rate was 69.2%, 50.1% and 33.6% in the months of May, June and July respectively. Dissolved oxygen (DO), Biological oxygen demand (BOD) and PH were found to significantly influence snail distribution in the site (p<0.05). The finding showed that 54.2% of all the snails collected were infected. The snail distribution observed may give an insight into some aspects of the epidemiology of trematodes infection in Okigwe the site for the study and their potential risks to infect human beings and animals.

It is important to examine such anthropogenic sounds and their effects upon aquatic animals, so that it is possible to introduce protective regulations. Fishes and other aquatic animals can detect underwater sounds and use them to obtain key information about the environment around them. Sounds travel rapidly over great distances in water and can provide detailed information on the presence of prey, predators, and related fishes, while the overall acoustic scene provides the fishes with key information about their environment. Although some of the background noise is generated by natural sources, including the precipitation of rain and snow, and wind and waves, many underwater sounds now come from anthropogenic sources, often termed “noise”. Some of these human-made sounds can kill or injure fishes and other aquatic animals, also impairing their hearing, and altering their behavior. There is a need for more work on the impact of human-made underwater noise upon the fitness of aquatic animals. This paper considers the gaps in information that must be resolved. The effects that need to be considered include death and injuries, physiological effects, and changes in behavior.

Antioxidant Activity of Biofield Treated Proprietary Test Formulation Supplemented with Vitamins and Minerals in Vitamin D3 Deficiency Diet (VDD) Induced Sprague Dawley Rats

A proprietary formulation was designed that consisted minerals (zinc, magnesium, iron, calcium, selenium, and copper), vitamins (pyridoxine HCl, cyanocobalamin, ascorbic acid, and cholecalciferol), Panax ginseng extract, and cannabidiol isolate. The study was aimed to assess the potential of the novel test formulation (blessed) and per se to the animals with the Trivedi Effect® in male Sprague Dawley (SD) rats, fed with vitamin D3 deficiency diet (VDD). The test formulation consisted above mentioned ingredients was divided into two parts. One part was left aside as the untreated test formulation without any Biofield Treatment, while the other part was defined as the Biofield Energy Treated sample, which received the Biofield Treatment by renowned Biofield Energy Healer, Mr. Mahendra Kumar Trivedi. The level of lipid peroxidation end product malondialdehyde (MDA) in liver tissues was significantly reduced by 34.59%, 34.91%, and 65.81% (p≤0.001) in test formulation treated with Biofield Energy (G5), Biofield Treated test formulation from day -15 (G7), Biofield Treatment per se with Biofield Treated test formulation from day -15 (G8) groups, respectively as compared to the disease control group (G2). Moreover, level of catalase enzyme in liver tissues was also increased by 8.64% in the G7 group as compared to the G2 group. Besides, in brain homogenate the level of glutathione peroxidase (GPx) was significantly increased by 433.94%, 266.97%, 133.94%, 467.89%, and 489.86% in the G5, Biofield Energy Treatment per se to animals from day -15 (G6), G7, G8, and Biofield Treatment per se animals plus untreated test formulation (G9) groups, respectively than G2. Antioxidant enzyme like superoxide dismutase (SOD) was significantly (p≤0.001) increased by 14.16% in the G9 group as compared to the G2 group. Allover, results signified that the Biofield Treated test formulation significantly increased antioxidative parameters, could be able to give support against oxidative stress induced by free radical and to maintain a good human health.

A novel proprietary test formulation was designed which included minerals, vitamins, β-carotene, cannabidiol isolate,and Panax ginseng extract. This present study was evaluated the impact of the Trivedi Effect® on novel proprietary test formulation in male Sprague Dawley rats, fed with vitamin D3 deficiency diet (VDD). The novel test formulation was divided into two parts; one part was defined as untreated test formulation, while the other part was defined as the Biofield Energy Treated sample, which received the Biofield Energy Healing Treatment by renowned Biofield Energy Healer, Mr. Mahendra Kumar Trivedi. The level of 25-OH Vit. D3 was measured in brain homogenate, which was found to be increased by 20.13%, 24.12%, 45.86%, 14.79%, and 29.96% in the G5 group treated with Biofield Treated test formulation, Biofield Energy Treatment per se to the animals (G6), 15 days pre-treatment of Biofield Energy Treated test formulation (G7), Biofield Energy Treatment per se plus Biofield Energy Treated test formulation from day -15 (G8), and untreated test formulation to the Biofield Energy Treated animals (G9) groups respectively, as compared with the disease control (G2) group. Brain acetylcholine (ACh) level was increased by 61.33% in the G7 group as compared with the untreated test formulation (G4) group. The expression of interleukin-6 (IL-6) was significantly reduced by 43.44% (p≤0.01), 30.93%, 21.42%, 45.99% (p≤0.01), and 60.85% (p≤0.01), respectively as compared with the G4. Lung pro-inflammatory cytokine tumor necrosis factor alpha (TNF-α) level was significantly reduced in the G5, G6, G7, and G8 by 24.86%, 32.55% (p≤0.01), 30.12% (p≤0.01), and 42.69% (p≤0.01), respectively, as compared with the G4 group. Altogether, the Biofield Treated test formulation and/or per se treatment to the animals significantly improved the levels of active form of vitamin D3 metabolite (25-OH Vit D3) and neurotransmitter (ACh); consequently significantly lowered the expression of proinflammatory cytokines (IL-6 and TNF-α). Therefore, the energized test formulation or per se treatment could be effectively useful against neuronal damage and inflammation for the management of brain disorders such as Alzheimer’s disease, dementias, brain cancer, epilepsy and other seizure disorders, mental disorders, and Parkinson’s. Thus, the results showed a significant slowdown of disease progression and all other disease-related complications/symptoms in the preventive Biofield Energy Treatment group per se and the Biofield Energy Treated Test formulation groups (viz. G6, G7, G8, and G9) as compared to the disease control group.

Effect of Hydroxychloroquine on Clinical Improvement and Mortality Among Patients with COVID-19 Admitted to Four General Hospitals in Saudi Arabia

Background The use of hydroxychloroquine in coronavirus disease (COVID-19) pandemic raised significant concerns as regards safety and efficacy in hospitalized patients. The objective was to examine the effect of hydroxychloroquine on clinical improvement and mortality among hospitalized patients with COVID-19. Methods A prospective cohort study was conducted at four general hospitals in the Western region, Saudi Arabia. Patients who had absolute or relative contraindication for using hydroxychloroquine were excluded. Patients concomitantly receiving other medications including azithromycin, antivirals, and supportive treatment were not excluded. Results A total 267 patients were included in the current analysis; 185 (69.3%) on hydroxychloroquine and 82 (30.7%) on non-hydroxychloroquine treatments. The average age was 46.0±13.3 years and 78.3% of the patients were males. Approximately 95.9% of the patients were symptomatic with mild (50.6%), moderate (32.6%), severe (8.2%), or ARDS symptoms (4.5%). Compared with no hydroxychloroquine, those on hydroxychloroquine had significantly longer length of stay (11.5±7.1 versus 7.8±4.3 days, p<0.001), more ICU admission (22.7% versus 9.8%, p=0.012), and more intubation (12.4% versus 3.7%, p=0.026). Improvement of symptoms (84.3% versus 81.7%, p=0.595) and hospitalization death (7.0% versus 1.2%, p=0.071) were not significantly different between groups. With exception of length of stay, the association of hydroxychloroquine with the above negative outcomes disappeared after adjustment for several factors including disease severity and concomitant use of azithromycin. Conclusions Hydroxychloroquine is not associated with better improvement of symptoms compared with other treatments. Moreover, it is associated with longer length of stay but not mortality or ICU admission in adjusted analysis.

Social Network Analysis of Integrated Medical Services for Hypertension – Using District Hospitals of Shanghai as a Model

Objective To examine the current linkage between different medical services for hypertension patients for enhanced integration among medical service systems. Methods A total of 18 hospitals and community medical centers from a district of Shanghai were enrolled for social network analysis which covered emergency visits and hospitalization records of 171,177 outpatients with hypertension. Stata software was used for data preprocessing and UCINET software was used for network analysis of medical service providers to quantify and visualize the network tightness and the "main role" of information delivery of the medical institution network in the area. Results The service network of hypertension consultation institutions in the region is closely connected as a whole, but the level of diagnosis and treatment of medical services in various communities varies widely, and the degree of association with higher-level medical institutions is not uniform. Conclusion Based on the limited tightness of various medical service providers, it is necessary to implement the responsibilities of individual medical institutions at different levels and pay more attention to improving the service capabilities of primary medical institutions for enhanced integrating medical services in future.

The experimental results obtained on the synthesis of materials from a melt in a solar furnace are presented. The features of the interaction of concentrated solar radiation with materials are analyzed. The mechanisms are described and a model for the interaction of concentrated solar radiation with materials is created. A feasibility study is proposed for the technology of glass-based glass materials obtained on the Big Solar Furnace.

Perception of Health Care Professionals on Transplantation in the Treatment of COVID-19 Patients

Corona virus disease 2019 (COVID-19) caused by severe acute respiratory syndrome corona virus 2 (SARS-CoV-2) has rapidly evolved as a pandemic with a challenge to the entire world for its management. Various modalities of treatment have been tried till date and when all the modalities failed then the only option that has been shown to be successful in some cases is lung transplantation. Decision for Solid-organ transplantation is not only made based upon its therapeutic requirement but also need to be supported by the law of land. In this regard, current Nepalese law is not with the provision for lung transplantation.  Thus, in order to make the concerned authorities aware of it and also as a step toward the preparedness for COVID-19 pandemic, this research has been conducted with an aim to see the perception of health care professionals of tertiary care centre of eastern Nepal regarding the legal aspects of lung transplantation. Conclusion Outcome of this research has supported the therapeutic aspect of transplantation over its legal issue in the emergency conditions like COVID-19 Pandemic.

Animals in the COVID-19 Era: Between Being a source, Victims, or Maybe our Hope to Overcome it!

The indirect misuse of animals usually correlated to catastrophic consequences like the one we are facing now, novel coronavirus disease-19. However, the source of infection is not fully confirmed yet. But the finger of blame points to bats and pangolins. Hence, understanding and raising awareness about wild animals and the consequences of their misuse are a must to avoid future pandemics. Herein, we threw light on the most animals involved in the current epidemic as well as possible animals in the shadow. Random serological investigations of samples from animals of different species (including asymptomatic animals) are required to understand the prevalence and severity of the infection, the extent of the transmission, and the monitoring of the situation over time. Moreover, alleviating fear associated with the possibility of human infection from their pets, we highlight that there are no reports that confirm this hypothesis; however, the opposite has been reported. Similarly, there is no proof that pigs can become infected amplify or spread novel coronavirus.

Knowledge, Attitudes and Practices of Prescribers towards Antimicrobial Stewardship at Hospitals in Khartoum State - Sudan

Introduction Antimicrobial stewardship (ASP) is of the utmost importance as a way to optimize the use of antimicrobials to prevent the development of resistance and improve patient outcomes. So, it is worthwhile to assess the knowledge, attitude and awareness regarding antimicrobial stewardship in hospitals. Objective The aim of this study is to assess knowledge, attitudes and practices (KAP) of prescribers towards antimicrobial stewardship at hospitals in Khartoum state and to identify the associations between prescriber’s demographic information and their knowledge. Methodology This descriptive cross-sectional study multi-centered study conducted in 10 hospitals at Khartoum state -Sudan, during period from November to December 2018. Study population included all prescribers who is available at study’s hospitals during study period and willing to participate in the study. A self-administered questionnaire addressing participants’ knowledge, attitudes, and practice (KAP) regarding antibiotic resistance and ASP distributed in the selected hospitals among attending house-officers, registrars and consultants completed then analyzed. Results Of the 294 medical staff targeted, 287 responded to the survey (response rate 97.6%). Only (26.4%) were familiar with the term ASP and (31.5%) claimed that it is effective in reducing resistance. (43.0%) of respondents believe that ASP play vital role on antibiotic prescribing. Only (9.5%) had ASP in their hospital and (13.5%) having policy and team. (45.3%) of participants had good level of knowledge about antimicrobial stewardship, but majority show negative attitude (63.1%), and poor practices (92.0%) regarding ASP. There was no observed correlation between knowledge and attitude, knowledge with practice (p-value ≥ 0.05). Only attitude with practice shows significance correlation (P=0.0001), which means that prescribers with positive attitude had the better practices towards antimicrobial stewardship. Age, occupation and experience are the only significant predictors of prescriber's knowledge and attitude towards antibiotic stewardship, while no association between these factors and practice. Conclusion The present study concludes that the knowledge of prescribers regarding ASP is moderate and their attitude is negative. Unfortunately, practices regarding ASP were poor, despite, the good knowledge regarding the effects of ASP on antimicrobial resistance.

A Study on Redox Potential of Phytochemicals and their Impact on DNA

The phytochemicals are endowed with antioxidant activities because of the presence of plentiful polyphenols and many other phytochemicals. However, some recent reports have suggested that phytochemicals from certain plant species exhibit DNA damaging properties mainly due to presence of alkaloids. In the present study, pBR322, Salmonella typhi DNA, insect DNA and human DNA were treated with hexanolic extract of Argemone mexicana and Thevesia peruviana leaves to assess their DNA damaging abilities. Another set of experiments was carried out using the methanolic extracts of Citrus lemon leaves to assess their DNA protecting abilities from damage of DNA by extracts of A. mexicana and T. peruviana at 150000 ppm for all DNAs used. The results indicated that the leaves extract of A. mexicanaand T. peruviana demonstrated significant DNA damaging potential at higher concentrations. In contrast, the extracts from C. limonat 15000 ppm showed maximum DNA protective properties for all DNAs used.

Assessment of Heavy Metals Contamination of Water and Cupscale Grass (Sacciolepisafricana) along the Epie Creek in Bayelsa State, Nigeria

This study was conducted to investigate the concentration of three heavy metals; iron, chromium and lead in surface water and plant (Sacciolepisafricana) tissues along the Epie creek. This was aimed at determining the heavy metal loading of surface waters and the extent of bioaccumulation in the most prevalent grass species found growing within the river course. Metals were determined using atomic absorption spectrophotometer. Result of water analysis recorded the least and most significant iron concentrations of 3.42 ± 0.06 mg/L and 12.06 ± 0.06 mg/L for Agudama and Edepie axis respectively. Contrastingly, water samples showed chromium and lead levels which were below their instrument measurable limits. Consequently, lead (Pb) and chromium (Cr) levels were within WHO permissible limits, while iron (Fe) exceeded regulatory standard for water samples collected from all field locations. Similarly, iron recorded elevated iron concentrations in plant (cupscale grass) with the least amount being revealed for Igbogene stem (196.63 ± 5.21 mg/kg), and the most concentration been reflected for Edepie root sample (9,423.17 ± 48.55 mg/kg). Even though iron levels prevalently exceeded recommended threshold in plants, the stem were mostly within limit for iron. The only grass sample depicting significant lead amount (0.34 mg/kg) was the stem section collected at Akenfa location. Evidently, metals were least stored in plant stems and most bioaccumulated in plant roots. Furthermore, Cr and Fe are the only significantly correlating metals in plants (r = 0.700, p < 0.01). Overall, the increasing level of Fe in the water environment is further indication of the impact of unregulated dumping of scraps and domestic waste along the creeks embankment.

Biolarvicidal Potentials of the Methanolic-Leaf-Extracts of Selected Tropical Plant Species

The global impact of malaria and challenges encountered during its control have necessitated the application of multifaceted strategies, including the application of plant-derived agents. Amidst these challenges the proliferation of the vector is becoming hyperendemic in tropical region. This research is focused on the biolarvicidal activities of the methanolic leaf-extracts of Cassia alata, Microdesmispuberula¸ and Spilanthesfilicaulis against mosquito larva. The mosquito larva were assayed in a static non-renewal test. Results showed no mortality for the negative control, and total mortality for the positive control (p<0.05). The C. alata bioassay was the most active with LC50 value of 13.73 ppm, followed by; M. puberula(21.24 ppm), and S. filicaulis (28.86 ppm). This study concludes that methanolic-leaf-extracts of C. alata, M. puberula¸ and S. filicaulis can be recommended for the formulation of biolarvicide for the control of malaria.

The Sustained and Selective Effectiveness of a Videophone Conversation for Individuals with Dementia

Studies reported that a videophone conversation allows individuals with dementia to be psychologically stable even after a 3 h conversation session. The individuals who exhibited difficulty in watching TV programs could still enjoy a conversation. This follow-up study investigated the effectiveness of a videophone conversation using five subjects with mild to moderate dementia. Two sessions were conducted during the study. In Session A, a subject convers with a talking partner through videophone for 30 min, and in Session B, a subject watches his/her favorite TV program. Sessions A and B were rotated on a day-by-day basis. Their psychological stability was evaluated by caregivers using the revised GBS scale (concomitant evaluation) and the overall psychological stability 3 h after each session (delayed evaluation) was also assessed. The significant psychological stability was obtained for two subjects in the concomitant evaluation, and one subject in the delayed evaluation. This indicates that a videophone conversation is a promising remote intervention for assisting individuals with dementia. Further investigation with the participation of more subjects must be conducted.

Ordeals of Sexually Violated Women and Access to Comprehensive Healthcare: A Case Study of Victims of Sexual Violence in North Kivu, Eastern Congo

Background: The impact of sexual violence in any community is extremely devastating and women in the Eastern part of the Congo are no exception. Sexual violence not only affects the health of women, but it impacts their social life within the community too. Objective: The study aims to investigate the experiences of female victims of sexual violence in accessing medical care in North Kivu. Design: An interpretive, phenomenological approach was used for this inductive and qualitative study. In-depth informant interviews were the main data collection tool. Open-ended questions were used during the interviews in order to garner more information from the interviewees. Heidegger’s approach was utilized in analyzing the collected data. Results: The analyzed and interpreted results of the data indicated that survivors of sexual violence are engaged in an ongoing struggle. The victims demonstrated immense resilience despite the lack of comprehensive medical care and have continued to reassemble their broken lives. In order to present the outcomes of the research in a succinct and coherent manner, the outcomes are categorized into five sub-themes: managing worries and shame; regaining happiness; healing and restoration; the need for professional assistance and struggles in daily life. Conclusion: The study provides an understanding of the recovery processes of survivors of sexual violence in North Kivu, with important insights into dimensions that rehabilitation programs should take into consideration.

Genetics alone cannot thoroughly expound the environmental impact on the molecular complexity of the endocrine system. Epigenetic-induced alteration in gene expression has emerged as a way in which environmental compounds may exert endocrine effects. The environmental compounds that interfere with normal endocrine signaling are one of the largest classes of toxicants we are exposed to, on a daily basis. Epigenetic mechanisms, mainly the methylation of DNA and the modification of histones, lead to differentiated activation and deactivation of genome domains creating phenotype plasticity and divergent endocrine function among populations and individuals, as well. The issues examined in the present review are related to environmental epigenetics, and more precisely, the epigenetic-mediated modulation and relevance of endocrine disrupting chemicals, focusing on three broad aspects: 1) persistence of EDs, 2) their major hormonal effects and 3) the potential of compounds previously considered as endocrine disruptors to induce epigenetic effects. Evidence suggests that environmental exposures notably impact expression of endocrine-related genes and, thus, affect clinical endocrine outcomes.

Comparative Anatomy of Mandibular Neurovascular Canals in Modern Human and Great Apes: A Pilot Study with Cone Beam Computed to Mography

The aim of the present study was to compare mandibular neurovascular canal anatomy in human and great apes by using cone beam computed tomography (CBCT). The anatomical variability of mandibular neurovascular canals (mandibular, incisive and lingual canals) of 129 modern humans and great apes (Homo, Pan and Gorilla) were analyzed by linear measurements on CBCT images. The Kruskal-Wallis non-parametric test and Dunn’s all pairs for joint ranks were applied to compare the variability of mandibular canals among these groups. Human, Chimpanzee and Gorilla groups showed significant differences in the dimensions of the mandibular canal, mental foramen, incisive canal, lingual canal and anterior mandibular bone width. Bifid mandibular canals and anterior loops were the anatomical variations most frequently observed in the Gorilla. Humans had a larger mental foramen and a distinctive incisive canal. The latter could not be identified in the Gorilla group. The variability in the anatomy within mandibles of human and non-human primates, shows different forms in the neurovascular structures. In comparison to the mandible of great apes, the incisive canal is suggested to be a feature unique to the human mandible.

Obstructive Sleep Apnea in Individuals with Down Syndrome: A Meta-Analytic Literature Review

Objective: Individuals diagnosed with Down syndrome (DS) are predisposed to obstructive sleep apnea (OSA). The aim of this study is to assess the prevalence of OSA and factors associated with OSA in this population. Study Design: Systematic literature review and meta-analysis. Methods: Studies of DS and OSA in the English language through May 2017 were reviewed. Since parental report of symptoms has limited validity related to the diagnosis of OSA, only studies using in laboratory polysomnography to diagnose OSA were included. Results: Twenty three studies examining OSA among 1,469 people with DS were found. Among ten studies using community referred samples, 71.5% of people with DS had OSA, compared to 69.6% in referred community samples suspected of having respiratory events. There was an inverse relationship between apnea hypopnea index (AHI) cutoffs and OSA prevalence as higher cutoffs were associated with somewhat lower prevalence. Examining age groups, adults had a higher prevalence of OSA (90.0%) compared to infants (66.5%) and children between 2-21 years of age (69.9%). Oxygen desaturation and gender did not affect prevalence. Although surgery had less effect on successfully treating OSA among DS individuals compared to those without DS in prior studies, lingual tonsillectomy had the greatest effect (mean AHI decrease of 9.0). Conclusion: OSA appears to occur frequently in children and adults with DS. Untreated, OSA may contribute to health problems and premature death, highlighting the potential importance of identifying OSA among people with DS. Even after traditional surgeries (i.e., adenotonsillectomy, tonsillectomy, adenoidectomy), repeating PSG is highly recommended as residual OSA can persist

Cyanobacteria have been implicated in the etiology of ALS for the past 50 years. The weakness of the theories of cyanobacteria or its neurotoxin, BMAA as the etiologic agent in ALS is the iniquitousness of cyanobacteria in the environment. In third world countries, clean water is far from commonplace, the exposure to cyanobacteria higher, yet the incidence of ALS is probably less than it is in developed countries. Even in the developed world, exposure to cyanobacteria is commonplace. Differences in the gut microbiome, possibly the presence of Proteobacteria, a protective agent against cyanobacteria toxins, may be important.

Turnover of Registered Nurses in Israeli Hospitals: A Secondary Analysis from a National Survey

Aim: To assess the extent and directions of internal hospital turnover of registered nurses and to examine the associated factors and stressors. Background: Internal turnover of hospital nurses is mainly horizontal. Occurring on a large scale, it could lead to dropout of skilled personnel, burdening hospitals both financially and in terms of human resources, affecting the quality of care. Methods: A secondary analysis based on data from the national study on "Patterns and trends of the nursing workforce in Israel". A structured telephone interview was conducted of 2,098 hospital nurses (October 2008-February 2009). Results: The rate of internal turnover between hospital departments was 29.7%. The main departments from which nurses moved were medical and surgical. The main departments from which nurses moved were oncology departments, Intensive Care Units, nursing administration. The major work-related stressor concerned salary (59%), two predictors of internal turnover were nurses with children under the age of 18 and academic degree nurses. Conclusions and Implications for Health Policy: Internal turnover rate in Israeli hospitals is moderate, with high rates in medical and surgical departments. It is vital to identify the main factors associated with high turnover rates and implement focused managerial retention strategies and other intervention programs.

Blood Donation and Misconceptions- A Nationwide Survey in Dental Hospitals of Pakistan

Background Blood donation is a common practice in many countries of the world and it benefits many of the people suffering from conditions which require blood transfusions. The blood donated should be appropriately screened for certain infectious diseases such as HIV/AIDS, Hepatitis B, Syphilis and Gonorrhea through serological tests. Materials and Methods This cross sectional study was conducted from January 2014 to August 2014, Data was collected from eight dental hospitals in four provinces of Pakistan. Attendants of the patients admitted in the surgical ward with age more than 16 years were included. Knowledge and practices of the participants were evaluated through a self-administered questionnaire. Ethical consent was obtained from the ethical committee or concerned authority of all eight hospitals. Results Response rate was 84 %. In total 55% male and 45 % female participated with 15.5% of the total sample population was under the age of 20 years. The age group with the maximum number of participant was between 20 to 39 years of age. 26% and 22.5% of the males and female participants respectively had knowledge. About blood donation, lesser percentage of attendants, 7.2% of the males and 4.6% declared that they had donated blood in the past. Based on education levels, it was highest among graduates (3.5%) and less in primary educated (2.2%) and uneducated respondents was 0.7%. 18% of the males and 17% females asserted that they hold some kind of fear of blood donation. Conclusion The population is not adequately knowledgeable regarding blood donation. There is a need for proper campaign to educate masses at national level within Pakistan to promote more people to become regular volunteers.

“On the Road to A Better Life”: An Innovative Suicide Prevention Program Based on The Realization of Meaningful Personal Goals

Background. Suicide prevention programs centred on reinforcing protective factors are uncommon. Aims. Since leading a meaningful life is incompatible with suicide, a 14-week program designed to help participants realize meaningful personal goals was created to improve the psychological well-being (PWB) of depressed older adults (≥ 65 years). Method. Persons scoring 9 or above on the Beck Depression Inventory- II (M = 22.05) took part in the program (n = 24). Their levels of PWB, depression, and suicidal thoughts were compared to those of a control group (n = 18). The questionnaires were completed three times: pre-test, post-test, and follow-up (six months later). Results. Analyses showed that the program participants improved significantly on most indicators of wellbeing, including suicidal ideation, indicating that increasing protective factors could be an innovative way to prevent suicidal ideation. However, the changes were not significantly higher than those observed in the control group, except for meaning in life. Conclusions. The absence of difference between groups was partly justified by unexplained improvements of controls between post-test and follow-up. Future studies should develop suicide prevention program thatincrease protective factors that provide individuals with means to achieve an optimal state of functioning.

Psychological “Risks” of Colonoscopy are Greater Amongst Fecal Immunohistochemical Test Positive Individuals than those with Inflammatory Bowel Disease

Background: Colorectal cancer (CRC) screening by Fecal Immunohistochemical Testing (FIT) followed by colonoscopy reduces colorectal cancer mortality. Barriers to colonoscopy should be minimised. Objective: To compare psychological “risks” of colonoscopy in FIT positive (FIT+) subjects and those with Inflammatory Bowel Disease (IBD). Method: IBD patients undergoing colonoscopic CRC surveillance were age and gender matched with FIT+ individuals awaiting colonoscopy. Subjects completed Spielberger State and Trait Scales for current levels of anxiety, depression, anger and curiosity, versus long term personality tendencies. Results: 70 IBD respondents were matched with 70 FIT+ respondents, (57% male, mean age 57.6 years). FIT+ subjects demonstrated greater scores for state Anxiety (22.3 vs 20.3 p=0.024), Curiosity (24.3 vs 21.8 p=0.036), Anger (13.7 vs11.5 p=0.037) and Depression (23.8 vs21.2 p=0.002). Conclusion: FIT+ patients experience more anxiety and depression prior to their colonoscopy than IBD patients, which may reduce colonoscopy uptake and is important to address.

Muscle Activation Signals During Gait Parkinson’s Disease are More Rhythmic than in Healthy Controls

Background To quantify the underlying rhythmic patterns observed in surface electromyography (SEMG) in patients with Parkinson’s Disease (PD), to compare rhythmicity of PD gait with normal gait, and to compare overground walking to treadmill gait. Methods Eight individuals with idiopathic PD, and ten individuals with no history of neurological disorders participated. SEMG was recorded from the vastus lateralis, tibialis anterior, lateral gastrocnemius and biceps femoris muscles bilaterally. Each participant performed two trials of overground walking and two trials of treadmill walking at a self-selected comfortable walking speed. SEMG was analyzed using a non-linear statistical model that identified underlying recurrent locomotor patterns, which estimated the relative contribution of central pattern generators to the observed muscle activation signals. An index of rhythmicity was determined from the statistic, R2. Results The rhythmicity of PD gait was significantly higher than that of normal gait (p = .0458). There was also a significant difference between the rhythmicity of overground walking and treadmill walking (p = .0097). Conclusions Individuals with PD appear to walk with muscle activation patterns that are more rhythmic than normal. This suggests that there is more stride-to-stride consistency, and there are fewer postural adjustments and responses to perturbations. We also found that treadmill gait was more rhythmic than overground walking. These findings, although preliminary, challenge the paradigm and current approach to gait retraining of patients with PD.

A High Volume, User-Friendly Immunization Clinic in Vellore, India: A Model for Urban Hospitals

The Expanded Program on Immunization (EPI) in India delivers vaccines under its policy, through government hospitals, health centers and outreach clinics. However, the national average full immunization coverage is stagnating at about 70%. The days and times of clinics are not always convenient for mothers working at home or employed outside. Moreover, vaccines not under EPI, but recommended by the Indian Academy of Pediatrics (IAP), are unavailable there, forcing mothers to go to private sector immunization clinics to get them. Recognizing the local need for user-friendly immunization clinic, we designed one in the Department of Child Health. The impetus came from local demand by a civil society organization (Rotary Club of Vellore). Our immunization clinic is open every working day (8.30 am to 4.00 pm) offering both groups of vaccines. Its success is illustrated by the fact that over 7000 children per month receive immunization in this clinic. We describe its design and operation so that this model may be replicated in other urban hospitals with pediatric sections.

Physical Activity and Risk Factors Screening for Ischaemic Heart Disease in South African Individuals Living with HIV

People living with HIV (PLWH) are at risk of developing chronic lifestyle diseases such as ischaemic heart disease (IHD). Physical inactivity is a modifiable risk factor for IHD. The level of ambulation physical activity in individuals living with HIV in a South African context is unknown. The aim of this study was to assess the physical activity levels and other risk factors for IHD in PLWH on antiretroviral therapy (ARV). An observational study was conducted from October 2010 to June 2012 at an outpatient clinic in Johannesburg, South Africa. Two hundred and five individuals who were on ARV for 6-12 months were screened. Physical activity was measured with the Yamax SW200 pedometer over a seven day period. Physical activity of the sample was reduced at 7673.2 (±4017.7) steps/ day with women walking less than men 6993.3 (±3462.6) and 10076.3 (±4885.6)respectively. Body mass index was increased to 25.6 (±5.4) kg/m2 with women noted to be overweight [26.6 (±5.5) kg/m2]. Independent predictors of being overweight were systolic blood pressure, waist and hip circumference, CD4 count and daily fruit and vegetable intake. Smoking was less common in the study population with 16.1% of the sample being current smokers and 25.9% former smokers. Individuals’ mean perceived stress levels were 19.9 (±7.8) on the Cohen’s Perceived Stress Scale. The ambulation physical activity level of individuals living with HIV requires modification to assist with reducing risk factors of IHD.  

Clinical and Immunological Beneficial Effects of Phyto V7 Consumption by HIV-1 Seropositive Individuals

Phytochemicals (PHT) are a large group of biologically active plant chemicals that may have positive effects on human health such as immune system stimulation, down regulation of inflammatory responses, radical scavenging activities, cell repair function, and antibacterial and antiviral activity. In this proof of principle 6 months study, the effects of supplementing a PHT mix, Phyto V7, to HIV-1 seropositive individuals and AIDS patients were examined. Individuals with CD4+ T-cells below 350 counts/mm3were assigned to one of the following treatments: CG1 - no treatment, CG2 - only highly active antiretroviral treatment (HAART), TG1 - only Phyto V7, and TG2- both Phyto V7 and HAART. After 3 months of treatment there were approximately (-)1%, 1%, 2% and 4% increase in the mean weight of the CG1, CG2, TG1 and TG2 groups, respectively. The tendency for the body mass index (BMI) was similar. The CD4+ counts increased by 13%, 39%, 53% and 35%, respectively. Similar trends were noted after 6 months with 2%, 79%, 53% and 69% increases in the CD4+ counts, respectively. There was a significant reduction in viremia only in groups receiving HAART. Overall better results were obtained in the group of patients receiving both HAART and Phyto V7, in which the mean weight increased by 5.7% and the CD4+ T-cell counts increased by 69% after 6 months. This study indicates that providing Phyto V7 to HIV-1 seropositive individuals and AIDS patients, receiving or not receiving HAART, improves their physical wellbeing and CD4+ counts, enabling them to cope better with the viral infection.

Anti-Oxidant Phytochemicals as Potential Treatments for Age-Related Macular Degeneration

Age-related macular degeneration (AMD) is responsible for a substantial proportion of severe visual impairment and blindness in people over 50 years of age. Current treatments for AMD are not effective in all patients and a proportion of patients who respond well to the treatment will still eventually develop central visual impairment. Despite all efforts to develop safe and efficient medications for AMD, as yet pharmacological approaches have failed to provide fully effective treatments for this condition. Various lines of evidence attest to the contributions of oxidative stress in the etiology of AMD. Anti-oxidant nutrients may be valuable preventive or therapeutic agents however complementary therapies can become widely adopted without sufficient knowledge of the real advantages and liabilities. This review considers the interventional potential of some common phytochemicals for treating AMD, primarily focusing on clinical and epidemiological evidence of potential public health relevance.

The Potentials of Antioxidant Micronutrients in the Management of Metabolic Syndrome

There is increasing evidence of the prevalence manifestations of metabolic syndrome worldwide. Metabolic syndrome is a cluster of abnormalities characterized by hypertension, central obesity, insulin resistance, endothelial dysfunction, dyslipidemia and oxidative stress. All these alterations predispose individuals to type 2 diabetes and cardiovascular disease that are major contributing factors to earlier mortality among people. The investigation of food nutrients that could reverse the features of metabolic syndrome is an important aspect for dietary-based therapies that may ameliorate the burden of the disorder. Antioxidant micronutrients are of great interest due to the recent described association between obesity, cardiovascular alterations and oxidative stress. These antioxidant nutrients are also being considered in the management of metabolic syndrome due to their potential benefits on hypertension, insulin resistance and hypertriglyceridemia since growing evidence has emerged that point to a causal link between oxidative stress and metabolic syndrome. Thus, dietary antioxidant supplements could have favourable effect on the attenuation and prevention of the manifestations of metabolic syndrome traits. Therefore, the present review focuses on the importance of antioxidant micronutrients in the treatment and management of metabolic syndrome.

Report of a False Positive Rapid HIV Test Due to Hepatitis A in a U.S. Army Soldier

A 25 year old, single, active duty soldier presented to a clinic in Afghanistan complaining of malaise, fatigue, acholic stools, and mild jaundice over a 5- to 7-day period. He had significantly elevated liver transaminase levels approaching 5000 U/L and a positive rapid human immunodeficiency (HIV) 1 antibody test. Ultimately, the patient was found to have a false positive rapid HIV-1 antibody test due to acute hepatitis A virus infection. This case report describes his evaluation and outcome, in addition to exploring possible causes of false positive HIV screening.

Random Forest Classifier for Respiratory Mortality Analytics

Respiratory diseases remain a major contributor to hospital morbidity and mortality worldwide, particularly among elderly patients and individuals with severe pulmonary compromise. Accurate prediction of respiratory mortality is clinically important for triage, resource allocation, ICU utilization, and early intervention. Traditional statistical models frequently demonstrate limited predictive sensitivity because respiratory mortality is influenced by complex interactions among demographic, diagnostic, physiologic, and severity-related variables. In this study, a machine learning framework was developed to predict in-hospital mortality among patients with respiratory disease using administrative and clinically derived variables, including age, sex, length of stay (LOS), diagnostic descriptions, risk of mortality and severity scores. A Random Forest classifier with balanced class weighting was developed and implemented to address nonlinear relationships and class imbalance within the dataset. Initial modeling demonstrated good overall discrimination performance, with receiver operating characteristic area under the curve (ROC-AUC) values approaching 0.84; however, mortality recall remained limited because deceased patients represented a minority class within the original dataset. To improve mortality detection, a physiologically informed synthetic augmentation strategy was developed. Synthetic clinical variables included oxygen saturation, ICU status, ventilator support, sepsis status, systolic blood pressure, creatinine, and lactate levels. Conditional physiologic consistency rules were incorporated during augmentation to preserve clinically plausible relationships among respiratory failure, hemodynamic instability, and organ dysfunction. The augmented dataset substantially improved model sensitivity and balanced mortality classification performance. Final model evaluation demonstrated strong predictive capability, achieving approximately 97% classification accuracy with balanced precision and recall across mortality classes. Confusion matrix analysis revealed marked reduction in false-negative mortality predictions compared with baseline modeling approaches. Feature importance analysis identified physiologic instability markers, respiratory severity classifications, LOS, and diagnostic respiratory categories as dominant predictors of mortality. These findings suggest that hybrid simulation-augmented machine learning frameworks may provide a valuable strategy for respiratory mortality analytics, particularly in datasets with limited real-world mortality prevalence and incomplete physiologic representation.

Implementing Evidence-Based Occupational Therapy for Multiple Sclerosis Rehabilitation: Exploring Knowledge on Barriers, Facilitators and Strategies

Background A persistent gap remains between evidence-based health care and its application in routine practice. This challenge is particularly prominent in allied health professions like occupational therapy (OT), where interventions are complex, individualized and centred on patients' daily functioning. Objective To identify barriers, facilitators, and implementation strategies for integrating evidence-based OT interventions in multiple sclerosis (MS) rehabilitation. Methods A mapping review was conducted using searches in five databases. Eligible studies included adults with MS, examined OT interventions, and reported on factors influencing implementation. Data were extracted and categorized using Grol’s framework for barriers/facilitators and Mazza’s taxonomy for implementation strategies. Results Fifteen studies met inclusion criteria. Barriers and facilitators were identified at multiple levels of Grol’s framework: 1-Innovation-level: accessibility, feasibility, and perceived attractiveness supported implementation, particularly when interventions incorporated holistic approaches, gamification, or printed manuals. 2-Professional-level: therapists’ competencies and alignment between interventions and patients’ priorities. 3-Patient-related: facilitators included motivation, readiness to change, and peer support, whereas fatigue, pain, and cognitive challenges served as barriers. 4-Social and organizational: effective teamwork, resource availability, and flexible scheduling facilitated successful adoption. 5-Economic or political: no studies. Implementation strategies focused on using manuals, patient empowerment, gamification, and organizational supports. No financial or structural policy-level strategies were identified. Conclusion Successful implementation of evidence-based OT for MS requires multifaceted, context-sensitive strategies addressing innovation, professional, patient, and organizational determinants. Practical approaches enhance uptake, while substantial gaps persist at economic and policy levels. Strengthening these areas may improve longterm integration and sustainability of evidence-based OT in MS rehabilitation.

Relationship Between Household Economic Status and Childhood Micronutrient Deficiency in India: An Evidence from a National Level Representative Survey

The high prevalence of anemia among children and adolescents in India is still an overwhelming problem. Not only that, there is also a considerable deficiency of various micronutrients such as Vitamin A, Vitamin B12, Vitamin D, serum ferritin, Zinc and Folate etc. in children. These micronutrients have several functional roles for the normal growth and development of children. Unfortunately, recent studies on public health and nutrition intervention have so far focused less on these micronutrition and more on anemia and nutrition. Data for this study obtained from the Comprehensive National Nutrition Survey (CNNS 2016-18), a nationally representative survey covering different age group. Specifically, it includes information on 9767 children aged 1-4 years. Out of these surveyed children biomarker data for hemoglobin, serum ferritin, zinc, folate, vitamin A, vitamin B12 and Vitamin D were collected from 8242 children. Micronutrient deficiencies were identified based on WHO and other established cut-off criteria. Wealth quintiles were computed to identify household economic inequality. The prevalence of anemia at the national level was 40.7%. Among micronutrients, folate (22.9%) and zinc (18.7%) deficiencies were most commonly observed, followed by vitamins A (18.3), vitamin B12 (13.8%) and vitamin D (14.0%). Iron deficiency as measured by ferritin was present in 31.6% of children. It is alarming that nearly one in three children (32.8%) suffer from deficiencies in two or more micronutrients. Clear socioeconomic disparities were observed for all micronutrient deficiencies (MND); children in the poorest groups had significantly higher levels of micronutrient deficiencies than children in the richest groups. Among the states, Gujarat and Madhya Pradesh had the highest overall micronutrient deficiencies, while West Bengal and Kerala had the lowest. This clearly shows that a large proportion of preschool children in India suffer from anemia and MND, with the prevalence being even more alarming in lower socio-economic settings. This study suggests that there is a need to move beyond single-nutrient interventions and implement comprehensive, multi-micronutrient supplementation or food fortification strategies seamlessly into the existing national health and nutrition programs.

Apomorphine Subcutaneous Infusion is More Efficient Than foslevodopa/foscarbidopa Subcutaneous Infusion in the Treatment of Advanced Parkison´s Disease for the Spanish National Health System

Objectives Motor fluctuations and non-motor disorders not manageable by first-line treatments in advanced Parkinson's disease require continuous dopaminergic stimulation strategies such as subcutaneous infusions of apomorphine (APO) or foslevodopa/foscarbidopa (FLD/FCD). A Budget Impact Analysis (BIA) was performed to estimate the cost difference between both treatments assuming equivalent clinical efficacy and safety. Material and methods The efficacy results of pivotal clinical trials at 12 and 52 weeks of treatment and the safety profile of APO vs FLD/FCD were compared, based on latest scientific publications and other available clinical data. A comparative BIA was performed, based on estimated annual drug treatment costs at Spanish published prices. Results The efficacy of APO (16 h/day) and FLD/FCD (24 h/day) in reduction of OFF hours (2.47 vs 2.75, 12 weeks; 3.66 vs 3.50, 52 weeks; respectively) and increase of ON hours without disabling dyskinesias (2.77 vs 2.72, 12 weeks; 3.31 vs 3.80, 52 weeks; respectively) could be considered clinically equivalent, as well as their safety profiles. However, a significant discrepancy is observed in the costs of the aforementioned alternatives. Considering published prices and the average dose reported in the literature, in Spain the annual cost of APO would be €13,980 compared to €55,198 for FLD/FCD. Consequently, the financial resources required for the treatment of FLD/FCD would enable the treatment of approximately three to four patients with APO. The BIA indicated the potential for annual savings in more than €2,500 million, considering a total target population of over 60,000 patients per year. Finally, an univariant sensitivity analysis was performed, considering a scenario in which the hospital acquisition cost of FLD/FCD decreased between 20%-30% (€44,159- €38,638/year). In this scenario, the total annual savings range between €1,875-€1,532 million per year. Conclusions Overall APO is more efficient than FLD/FCD, as it provides similar clinical efficacy at a lower treatment cost. The selection of an appropriate treatment option is to be determined by clinical criteria and patient characteristics, but cost evaluation should be considered to select the most cost-effective therapeutic option.

Climate Change Sustainable Net-Zero Emissions by Uptake Atmospheric Carbon Dioxide by Terrestrial and Aquatic Plants

A sustainable global net-zero carbon emission is when emissions are equivalent to the uptake of carbon by global terrestrial and aquatic plants. To meet that objective, an approximate 50% reduction of fossil-fuel emissions was estimated for net-zero by 2050 using carbon dioxide concentrations and mass emission monitoring by NOAA and related references. From that data, the initial target level of reduction to obtain net-zero was selected for year 2000 to minimise effects of climate change on environmental damage. The recent finding that about half global emissions are taken up from the atmosphere and increases global terrestrial and aquatic plant growth, including agriculture plants for food production, indicates it is important to maintain the year 2000 levels in the atmosphere. It was concluded as renewable energy was found to need support, it could be supplemented by electricity production remaining after obtaining net-zero, which also supports the atmospheric carbon dioxide levels for their important global ecological contribution.

Intravascular Laser Irradiation of Blood (ILIB) on Sleep Quality Improvement: A Randomized Placebo-Controlled Clinical Trial

Introduction Sleep quality is a fundamental determinant of human health and well-being. Modified Intravascular Laser Irradiation of Blood (ILIB), a non-invasive therapeutic modality, has emerged as a potential intervention for sleep-related disturbances. Proposed mechanisms include reduced blood viscosity and platelet aggregation, activation of superoxide dismutase, increased oxygen bioavailability, enhanced microcirculation, elevated serotonin levels, and decreased cortisol concentrations—physiological processes intricately involved in sleep regulation, mood modulation, and the stress response. Objective To evaluate the effects of Modified Intravascular Laser Irradiation of Blood (ILIB) on sleep quality in individuals with self-reported sleep disturbances. Methods A randomized, placebo-controlled clinical trial was conducted with participants who reported poor sleep quality. Subjects were randomly assigned to one of two groups: the intervention group received ILIB using a 660 nm red laser, while the control group received a placebo treatment (light emission with sub-therapeutic power, <1 mW). Both groups underwent the same treatment schedule. Sleep quality was assessed at baseline and after six treatment sessions using the Pittsburgh Sleep Quality Index (PSQI) and the Epworth Sleepiness Scale (ESS). Results Participants in the ILIB group showed statistically significant improvements in the primary outcome of global sleep quality. PSQI scores decreased from 10.24 at baseline to 6.47 post-treatment. ESS scores showed a non-significant change from 10.44 to 10.12. These results suggest enhanced overall sleep quality and reduced sleep latency, although the observed reduction in daytime sleepiness did not reach statistical significance. Conclusion Modified Intravascular Laser Irradiation of Blood appears to be a promising non-invasive approach for improving sleep quality. The clinical outcomes observed are comparable to those reported in both pharmacological and behavioral sleep interventions, particularly in terms of PSQI improvements. These preliminary findings support the need for further research to clarifyunderlying mechanisms, optimize treatment parameters (e.g., dosimetry and duration), and expand outcome assessments to include biomarkers and polysomnographic data.

Perceptions and Lived Realities of Women’s Engagement in Environmental Conservation in Patriarchal Communities of Northern Uganda: A Qualitative Inquiry

Background Women’s participation in environmental conservation within patriarchal societies is a critical factor for achieving sustainable development and improved community outcomes. Despite this importance, limited research has examined women’s perceptions of their roles and contributions to environmental conservation. The current study aimed to (1) explore women’s perceptions of their roles in environmental conservation within patriarchal structures, and (2) to document the effectiveness of strategies enhancing their participation. Methods This study followed a phenomenological design, using thematic data analysis. Information was gathered through semistructured interviews, focus group discussions (FGDs), and key informant interviews with technocrats, civil servants, local leaders, and NGO staff. The sample size of the study was 30 participants, obtained using the point of saturation principle. The focus group consisted of 24 participants and 06 Key Informants. The study tools were pretested with a small group before the main fieldwork. Participants were selected using purposive sampling. The size of the sample was guided by data saturation. Results The findings show that although women’s representation in local councils and environmental committees has grown due to the 30% quota requirement, their influence remains constrained by systemic barriers. Political interference undermines technical teams’ conservation efforts, while widespread community ignorance of environmental laws limits compliance. Weak enforcement and corruption, with leaders themselves implicated in destructive practices, further erode progress. Critically, inadequate financing such as subcounties allocating only 600,000 shillings for sensitization, leaves many activities unimplemented, forcing reliance on development partners. Conclusion The study concludes that sustainable environmental stewardship requires not only legal reforms and institutional accountability but also adequate financing and communitydriven strategies that amplify women’s voices and foster collective responsibility.

Synchronization of Transmembrane Na+ And K+ Traffic in the Epidermis is Evidence of the Criticality of Na+/K+-ATPase Function

Hair spectrometry results for sodium and potassium content in healthy individuals (n = 9991) were analyzed using mathematical statistics. It was found that the vast majority (81.6%) of the overall group had a (Na)/(K) ratio > 1, with a median (Me) of 2.35. However, 18.4% had a ratio < 1 (Me = 0.68). The median (Na)/(K) for the overall group was 1.98. Of particular interest was the analysis of the distribution of (Na) and (K) depending on the (Na)/(K) ratio. The results of this analysis are presented in the article.

A New Model of Body Composition: Concept and Design Features of the DBA-Model

Indices, based on data such as height and weight in general and in particularly the body mass index (BMI), are often used to assess overweight. However, there is limited capacity to differentiate the amount of fat mass between individuals. This review refers to an anthropometric model called Dahlmann-Body-Analysis (DBA), which uses simple anthropometric parameters to define a Reference Weight (Ref-Wt). It is based on hand circumference as a proxy for the skeletal frame and, in addition, the circumference of the abdomen as a proxy for central obesity. Processed through a network of algorithms, the DBA model enabled to differentiate the Difference Weight – that means the difference between the Actual Weight and the Reference Weight – into fat mass and skeletal muscle mass. The DBA-model resembles the 2-component model of Albert R. Behnke, which he considered as a living functional construct including essential fat. The DBA-model matches with Behnke`s 2-component model insofar, as the essential fat is replaced by a physiological amount of fat tissue. The review summarizes studies to compare DBA-derived data with Metropolitan Life Insurance tables, evaluates DBA-derived fat tissue mass with bioelectrical impedance analysis (BIA) derived results and analyses the meaning of the DBA model in clinical settings to uncover the underlying mechanisms of metabolic syndrome (MetS) pathogenesis with increasing amounts of fat mass. The model offers the opportunity to calculate changes in fat or muscle tissue in an absolute (kg) or relative (%) amount on individuals. The data suggest that the DBA-model has satisfactory prediction qualities for use as a practical tool in public health care.

Diabetic Mouse Delayed Wound Model Following Treatment with the NerveStim™ Neuropathy System

Delayed wound healing in diabetes is characterized by impaired angiogenesis, persistent inflammation, extracellular matrix dysregulation, and peripheral neuropathy. A preclinical study was conducted using a diabetic mouse delayed wound model to evaluate the surrounding tissue of a wound, (its periwound) and its tissue responses following treatment with the NerveStim™ Neuropathy System, a combination topical gel and neuromuscular electrical stimulation platform. Periwound tissue was harvested at Day 14 and analyzed using NanoString gene expression profiling. Treated animals demonstrated visibly increased periwound tissue thickness compared to untreated controls. Differential expression analysis identified 76 significantly upregulated and 17 downregulated genes. Upregulated pathways included angiogenesis (Vegfa, Fgf2, Pdgfb, Nos3), neurotrophic signaling (Ngf, Bdnf, Scn9a, Trpv1), macrophage polarization (Arg1, Mrc1, Il10), and extracellular matrix remodeling (Col1a1, Col3a1, Mmp9, Timp1). Downregulation of select pro-inflammatory mediators (Nos2, Mif) was observed. These coordinated transcriptional changes are consistent with activation of reparative immune, neurovascular, and matrix remodeling pathways in diabetic periwound tissue.

Characterization of People Receiving 2-Drug Regimens (2DR) for HIV Management in Italy

Objective To describe the clinical features and real-world treatment of people living with human immunodeficiency virus (PLHIV) using fixed-dose or free combinations of 2-drug regimens (2DR) of antiretroviral therapy (ART). Design Italian retrospective cohort study. Methods Data were extracted from PLHIV who initiated or switched to 2DR: Group 1 (fixed dose), Group 2 (free combination). Results Group 1 was younger and more predominantly male, and had shorter time from AIDS-defining diagnosis to 2DR-ART and from diagnosis to baseline, a lower prevalence of resistance, and fewer comorbidities than Group 2. Median baseline viral load was <50 copies/mL in both groups, but Group 1 had a higher mean due to outliers. The most common ART classes before switching to 2DR were Integrase Strand Transfer Inhibitor (INSTI)-based (48.97%), Non-Nucleoside Reverse Transcriptase Inhibitor (NNRTI)-based (22.73%), and Protease Inhibitor (PI)-based (16.53%). Distribution varied: Group 1: INSTI-based (53.13%), NNRTI-based (24.31%), and PI-based (15.04%); Group 2: INSTI-based (29.41%), PI-based (23.53%), and NNRTI-based (15.29%). After switching, Group 1 was on dolutegravir/lamivudine (79,33%) and dolutegravir/rilpivirine (20,67%); Group 2 mostly on INSTI-PI (52.81%), followed by NNRTI combinations, mainly with doravirine (19.10%). Duration of ART after switching was shorter in Group 1. Conclusion Italian PLHIV on 2DR fixed-dose combinations were younger, virologically suppressed individuals at baseline, with a shorter lead time from diagnosis, lower prevalence of resistance and lower comorbidity rate compared to those on free combinations. These findings underscore an unmet need for 2DR fixed-dose combinations, as the free combinations were predominantly utilized for more challenging populations.

Efficacy and Safety of Pulsed Magnetic Therapy in Sleep related Disorders: A Remote, Randomized, Double-Blind, Placebo-Controlled Trial

Background/Aim Sleep disturbances are common and are associated with impaired daytime functioning, reduced quality of life, and increased health risks. Non-pharmacological neuromodulatory interventions have gained interest as alternatives to hypnotic medication. Pulsed electromagnetic field (PEMF) therapy has been proposed as a non-invasive approach to improve sleep quality and recovery, but evidence from large, well-controlled trials remains limited. The objective of this study was to evaluate the efficacy and safety of pulsed magnetic field therapy (PMT) on sleep quality, sleep-related symptoms, daytime functioning, and well-being in adults with heterogeneous sleep complaints.  Materials and Methods In this remote, randomized, double-blind, placebo-controlled trial, 217 adults reporting non-restorative sleep, difficulties initiating sleep, or frequent nocturnal awakenings were assigned to active PMT using a PEMF device (Night Harmony Female/Male protocol) or a sham application for three weeks. Primary outcomes were changes in the Pittsburgh Sleep Quality Index (PSQI) global score and self-rated sleep satisfaction, restfulness, and sleep initiation difficulties. Secondary outcomes included sleep diary measures, daytime functioning, well-being (WHO-5), symptom burden (MYMOP), and daytime sleepiness (Epworth Sleepiness Scale). Analyses followed the intention-to-treat principle using ANCOVA with baseline values of the respective parameter as covariate.  Results Both groups showed significant improvements over time in global sleep quality and most secondary outcomes (p < 0.001). Between-group differences in PSQI global score were not significant (p = 0.314). However, active PMT resulted in significantly greater improvements in subjective sleep satisfaction (p = 0.02) and restfulness (p = 0.02), particularly among participants with moderate to severe baseline sleep disturbances (p < 0.001 for both). Sleep continuity measures improved similarly in both groups. No serious adverse events were reported.  Conclusions PMT produced modest but significant improvements in subjective restorative aspects of sleep beyond placebo effects and was well tolerated. Further studies using objective sleep measures and longer follow-up are warranted.

A Review and Update on the Biochemical Basis of Conscious Breathing (Pranayama)

Breathing has long been recognised as a vital process in Eastern traditions such as YOGA and Ayurvedic medicine, where Pranayama is regarded as a transformative practice that harmonises body and mind. While the East developed sophisticated systems of breath regulation for health and self-awareness, the West largely overlooked their significance until the late 19th and early 20th centuries. A group of pioneering physicians, sometimes referred to as "the pulmonauts", including Christian Bohr, Konstantin Buteyko, and Peter M Litchfield, explored the physiological foundations of respiration. They emphasised the benefits of slow, nasal breathing and its relationship to both physical and emotional well- being. In the 21st century, contemporary researchers and practitioners such as James Nestor, J. Patrick McKeown and Anders Olsson have expanded this understanding through the fields of biochemistry, physiology, and biomechanics. Their work has clarified the mechanisms underlying conscious breathing and refined ancient pranayama techniques. Integrating insights from both Eastern and Western traditions reveals that breathing is a central factor in maintaining homeostasis and emotional balance.

Uncovering the Gaps and Linkages: Exploring Postpartum Mood and Anxiety Disorders and Experiences for Clinicians and Parents within One Northern Community

The current research assessed gaps and barriers related to perinatal mental health service access by parents during COVID-19, as well as service providers’ knowledge and provision of PMAD services, in one Northern community, where rural and remote healthcare is a daily reality, and recruitment and retention of qualified professionals is a significant issue. Two mixed-methods surveys were designed, one to assess PMAD knowledge and service provision via a volunteer sample of service providers, and another to assess PMAD service need and access among a volunteer sample of new parents. Results indicated that many parents waited too long for services, and some parents in need of PMAD services didn’t receive them. Interestingly, higher-income parents were more likely than lower-income respondents to desire services and support for PMADs. Although most providers screened for, were trained in, and treated and/or referred patients with PMADs, they reported feeling as though they were working in silos, unaware of what other providers offered.

Lineage-Specific Disruption of Hematopoiesis by Oxaliplatin: Mechanisms of Erythropoietin Resistance and Immune Suppression

Background Oxaliplatin, a widely used chemotherapeutic agent, is associated with hematologic toxicities such as anemia, leukopenia, and thrombocytopenia. Despite their clinical relevance, the molecular mechanisms underlying lineage-specific bone marrow suppression remain poorly understood. Methods We administered oxaliplatin to mice over eight weeks and performed RNA-sequencing (RNA integrity >8) on bone marrow alongside peripheral blood analysis and cytokine profiling. Transcriptomic data were analyzed to identify differentially expressed genes (DEGs) and enriched pathways. For that, we applied a thematic Gene Ontology (thematicGO) enrichment method that groups GO terms into biologically meaningful categories, such as hematopoietic lineage disruption, cell cycle arrest, and cytokine signaling. Results Oxaliplatin induced broad transcriptional suppression of erythropoiesis and lymphopoiesis, with 3,691 DEGs identified (FDR<0.05, |FC|>1.5). Upregulation of Cdkn1a and downregulation of E2f2 suggest G1/S cell cycle arrest, correlating with repression of key erythroid maturation genes (e.g., Spta1, Slc4a1, Alas2) and hemoglobin subunits (Hba-a1/2, Hbb-bs/t). Despite a ~3000-fold increase in renal Epo expression, bone marrow Epor was reduced, indicating erythropoietin resistance. B and T cell markers were also significantly downregulated, signifying a collapse in adaptive immunity. Notably, neutrophil populations were largely spared. Cytokine analysis in plasma revealed a pro-inflammatory shift with elevated TNF-α and reduced TGF-β, potentially exacerbating hematopoietic dysfunction. Conclusions Oxaliplatin induces a lineage-dependent suppression of hematopoiesis, driven by coordinated cell cycle arrest, metabolic stress, and disrupted cytokine signaling. RNA-seq analysis enabled integration of transcriptomic findings into coherent biological themes. These findings provide mechanistic insights into oxaliplatin’s hematologic toxicity linking bone marrow failure (potentially reversible) via interconnected inflammatory and metabolic pathways and may inform therapeutic strategies to minimize or restore myelosuppression in cancer patients.

Activation of Transcription Factor EB Protects Against Proinflammatory Insults Through NF-κB Inhibition in Keratinocytes

Keratinocytes are pivotal in mediating cutaneous inflammation. Identifying anti-inflammatory factors within these cells holds promise for developing novel therapeutic strategies to manage skin inflammation. Transcription factor EB (TFEB) has recently emerged as a key regulator linking cellular energy metabolism to inflammatory processes, primarily through its influence on autophagy and NF-κB signaling. However, whether TFEB activation exerts anti-inflammatory effects in keratinocytes remains unclear. In vitro inflammation model was established in HaCat cells by incubation with proinflammatory mediators LPS and IL-1β. Cell viability and TFEB expression and phosphorylation were measured. The effect of TFEB activation by C1 and adenoviral TFEB overexpression on the expression of proinflammatory genes including COX-2, MCP-1 and IL-6 were detected. Also, IκBα protein level were determined. TFEB phosphorylation is increased while TFEB total protein expression is inhibited by treatment with LPS and IL-1β. Pharmacological activation of TFEB by compound C1 and TFEB overexpression suppressed the expression of COX-2, MCP-1 and TNF-α induced by LPS and IL-1β. TFEB overexpression increased basal IκBα expression and restored IκBα level under LPS treatment. TFEB knockdown reduced TFEB expression and lowered basal expression level of COX-2, MCP-1 and TNF-α. Our findings indicate that TFEB activation can mitigate inflammatory gene expression in keratinocytes triggered by LPS and IL-1β. This implicates TFEB as a significant novel modulator of cutaneous inflammation, highlighting its potential as a therapeutic target. Targeting TFEB could thus be a viable strategy for developing new treatments for chronic inflammatory skin conditions.

On Terms: Maladaptive Behavior

There is no shortage of terms that are used to describe and articulate the strategies, practices, and underpinnings associated with the work engaged in by behavior analysis. Two terms that a clinic-focused behavior analyst may hear often are maladaptive behavior and problem behavior. These terms are not only common within our everyday practice, but they are also peppered throughout behavioral literature. Despite their permanency in the behavior analyst's repertoire, their meanings remain inconsistent, interchangeable, and conceptually unclear. For over a century, this lack of precision has gone unchecked, despite extensive research on undesirable or clinically significant behaviors. This article offers individuals a recount of the history, definitional limitations, and functional implications of these terms. From the perspective of early psychological scholarship and contemporary behavior-analytic sources, I argue that the field of Applied Behavior Analysis (ABA) will benefit from distinguishing maladaptive behavior as the presence of behavior that adversely affects the individual, while problem behavior should refer to the absence or inhibition of adaptive responses in relation to others' social behaviors. Furthermore, I propose that both terms be used to describe the effects on the person engaging in the behavior, instead of others around the individual, as a method of improving directionality and functional clarity. As a field focused on verbal behavior, becoming more conceptually precise with our own verbal behavior will lead to a more coherent basis for interpreting and treating behaviors that interfere with individuals' adaptation and well-being.

A Decision Tree Ensemble Approach to Diabetes Prediction using the Framingham Heart Dataset, Exploring the Role of AI-Associated Interventions in Reducing Diabetes-Related Adverse Outcomes Between Men and Women

Objective Diabetes poses significant public health challenges, with many individuals remaining undiagnosed and at risk of complications. This study aimed to evaluate the performance of decision tree ensemble methods for predicting diabetes onset using the Framingham Heart Study Teaching Dataset and to explore sex-specific risk patterns relevant to AI-driven interventions. Methods We analyzed data from 11,627 participants, incorporating demographics, vital signs, smoking status, medication use, and laboratory measures. Random Forest classifiers were developed to predict diabetes incidence at approximately 6-year (Period 2) and 12-year (Period 3) follow-ups. Class imbalance was addressed using undersampling, oversampling, and the Synthetic Minority Over-sampling Technique (SMOTE). Results The models demonstrated robust performance, achieving an Area Under the Curve (AUC) of 0.856 in Period 2, and moderate predictive ability in Period 3 (AUC = 0.732 in males, 0.786 in females). Key predictors included glucose level, BMI, systolic blood pressure, age, and heart rate. Notably, differences emerged in predictive accuracy between men and women, suggesting potential sex-specific vulnerabilities that merit further study. Conclusion Machine learning approaches, particularly Random Forests, show promise for medium- and long-term diabetes risk prediction, supporting early identification and intervention efforts. Future work should focus on hyperparameter tuning and explainability techniques, such as SHapley Additive exPlanations (SHAP) values, to improve model precision, interpretability, and fairness. Equity-focused strategies remain critical to ensure AI-driven tools benefit diverse populations and do not exacerbate existing disparities in diabetes care.

Maxillary Sinus Lift Using the Lateral Window Technique with Hydroxyapatite and Beta-Tricalcium Phosphate (β-TCP) Graft

This narrative literature review investigates the clinical feasibility of maxillary sinus lift using the lateral window (or traumatic) technique, employing a grafting material composed of hydroxyapatite associated with beta-tricalcium phosphate. The study is based on the premise that bone resorption and sinus pneumatization—common in edentulous posterior maxillae—pose a challenge to achieving primary stability during dental implant placement. Given the limitation imposed by reduced residual bone height, bone-grafting surgical techniques become necessary to enable implant-supported rehabilitation. The objective of this work is to analyze, through a literature review, the efficacy of combining synthetic biomaterials as an alternative to autogenous bone grafts, which are considered the gold standard in implant dentistry. A bibliographic search was conducted in the PubMed and LILACS databases and through the VHL portal, prioritizing articles addressing the biological properties of alloplastic grafts, the indications of the lateral window technique, and the clinical success rates of implants placed in previously grafted areas using such materials. The findings demonstrated that hydroxyapatite associated with beta-tricalcium phosphate exhibits favorable osteoconductive characteristics, such as adequate porosity and gradual resorption, while allowing the formation of viable bone within a clinically acceptable timeframe. The combination of these materials eliminates the need for a second surgical site, reduces morbidity, and maintains treatment predictability. It is concluded that the lateral window technique associated with synthetic biomaterials is a safe and effective alternative for patients with maxillary bone atrophy, capable of promoting suitable bone beds for dental implant placement and stabilization, with lower surgical risk and satisfactory clinical outcomes.

Assessment of Nurses' Knowledge, Attitudes, And Willingness Towards Organ Donation: A Cross- Sectional Study at Tibebe-Ghion Specialized Hospital, Bahir Dar, Ethiopia, 2023

Objective To assess knowledge, attitude and willingness towards organ donation of nurses in Tibebe-Ghion Specialized Hospital, Bahir Dar, Ethiopia, 2023. Method A hospital based cross-sectional study was conducted from March 1 to May 30, 2023, using a census of all eligible nurses (N=215). Data were collected with a structured questionnaire adapted from validated tools. Binary logistics regression was used to identify factors associated with willingness to donate, with results presented as adjusted odds ratios with 95% confident intervals. Results The overall response rate of this study was 98% (n=206). The mean age was 29.5 (±4.8) years, and 55.8% were male. Overall, 61.7% (95% CI: 54.9-68.5) of nurses had good knowledge, and 45.6% (95% CI: 38.8-52.5) had a favorable attitude. In multivariable analysis, female sex (AOR=3.50, 95% CI: 1.18-3.92, p<0.001) and greater years of experience (AOR=2.15, 95% CI: 1.18-3.92, p=0.012) were independent predictors of willingness. Conclusion While a majority of nurses were willing to donate organs, significant gaps in knowledge and legal awareness persist. Female nurses and those with more experience were more willing to donate. Targeted educational interventions are recommended to address knowledge deficits and foster a more supportive environment for organ donation advocacy.

Background Research and drug development industries have multiphase drug screening procedures, which can be debated. As a result, harmful products may still reach for public health service delivery due to vulnerabilities in the process. Main body A wide range of test compounds have delayed manifestation of undesired effect on the study subject, with the time to undesired effects after acute exposure being weeks and months. Acute toxicology in a preclinical trial also has limited clinical value as its lethal dose is the endpoint for a conclusion, and death sometimes occurs after a scheduled period of acute toxicology. Countless resources are wasted, and numerous new drugs are introduced into the pharmaceutical market with assumed safety analysis every year due to vulnerable multi-procedures in preclinical trials. The principal use of collected data from a preclinical trial is to support regulatory categorization and harmful labelling decisions. However, the data can also be used to derive safe use threshold levels, which may lead to the use of unsafe material. The criteria for classification and labelling also differ among countries, sometimes among authorities within the same country. The fundamental concept of toxicology states that ‘all chemical substances are potential poisons depending on the amount and duration of exposure. However, the toxic property of a test compound cannot be created or eliminated by simply the amount administered to study animals. Conclusion All xenobiotics are poisons at any amount with different severity that can be calculated using biological parameters.

L-Carnitine (Lc) acts as an antioxidant that neutralizes free radicals, especially superoxide anions and protects cells against oxidative damage-induced apoptosis, as following ovulation, intracellular reactive oxygen species (ROS) accumulation increases in oocytes, Oocytes exhibit an intracellular defense mechanism against an oxidative attack. This outcome adversely affects fertilization and subsequent embryonic development, thereby increasing the risk of an early miscarriage and abnormal development of offspring. The purpose of this study was to see how adding LC to either maturation or fertilization medium affected the developmental competence of immature bovine oocytes. In this study, Ovaries from apparently normal reproductive organs of cattle were collected within 30 minutes from slaughter and evisceration of animals. Cumulus oocyte complexes (COCs) were collected by aspiration of medium sized ovarian follicles (4-8 mm). COCs of acceptable quality were selected, washed and incubated in tissue culture media 199 (TCM199) supplemented with 10% heat inactivated fetal calf serum, 5 μg/ml luteinizing hormone (LH), 0.5 μg/ml follicle stimulating hormone (FSH) and 1 μg/ml estradiol-17β for 20:22 hour at 38.5 C◦ under 5% CO2 in air with 90% humidity. different concentrations of LC (1.25,2.5 and 5mM) were used. The results were consistent for both maturation and fertilization and there is a significant increase in maturation, fertilization., cleavage and blastocyst rate. In conclusion, LC has important role in IVEP through addition of LC to maturation media or culture media it improved nuclear maturation and blastocyst formation rates in bovine oocytes.

Bipolar disorder is a chronic condition marked by episodes of mania and depression, significant functional impairment, and challenges with treatment adherence. Current guidelines highlight the importance of both medication and psychosocial approaches in treatment. This review explores the primary psychosocial interventions for bipolar disorder. Psychoeducation helps recognize early symptoms, improves medication compliance, and prevents relapses. It is simple to implement and cost-effective. Family-Focused Therapy (FFT) enhances family communication, reduces emotional expression, and lowers the frequency of depressive episodes. Interpersonal and Social Rhythm Therapy (IPSRT) supports maintaining social stability by addressing disturbances in biological rhythms. Cognitive Behavioral Therapy (CBT) decreases depressive symptoms and boosts treatment adherence by restructuring automatic thoughts. Additionally, cognitive and functional rehabilitation programs improve attention, memory, and executive functioning. Peer support groups and digital e-health tools, though supportive, have limited evidence of effectiveness. In summary, multicomponent psychosocial interventions serve as a valuable addition to medication, helping to prevent relapses, improve functioning, and enhance quality of life in individuals with bipolar disorder.

Reproductive Ability of Currant–Gooseberry Hybrids After Cryopreservation of Pollen in Liquid Nitrogen

The present study of the reproductive ability of currant and gooseberry hybrids after long-term cryopreservation of pollen in liquid nitrogen is of enormous importance for organizing the conservation of the diversity of this crop. Four distant interspecific hybrids of currant and gooseberry served as objects of the study. The pollen fertilizing ability was studied by pollination of allotetraploid Jošta with pollen of tetraploid hybrids stored for a year at ultra-low temperatures (–196°C) and freshly collected pollen (control pollination). Berry set in 2022, when pollinated with cryopreserved pollen, averaged 56.4% and was higher than in the control pollination (38.2%). In 2023, the number of berries set on average for all cross combinations in the experiment was also quite high (59.6%), but lower than in the pollination control (81.4%), which can be explained by unfavorable weather conditions during pollen collection in the year of establishment (2022). The obtained data specify that pollen of the allotetraploids does not lose its high fertilizing ability during its long-term cryopreservation. Berry setting, average berry weight and seed production are quite high and comparable to pollination with freshly collected pollen (control pollination variant). Seed germination and values of morphometric parameters of seedlings obtained from pollination with cryopreserved and freshly collected pollen in most cases have close values. Cryopreservation of pollen of currant-gooseberry hybrids for a year and more allows to preserve pollen for successful pollination and berry formation and is a reliable way to preserve germplasm.

Prevalence and Risk Factors of Intestinal Parasitic Helminths among Primary School Children in Oru West Lga, Imo State, Nigeria

Intestinal parasitic helminths pose a significant public health burden, particularly among children. This cross-sectional study investigated the prevalence and risk factors of intestinal parasitic helminths among 550 school children aged 4-12 years in Oru-West L.G.A, Imo State, Nigeria. Stool samples were examined using parasitological techniques, and questionnaires were administered to collect data on risk factors. The overall prevalence of intestinal parasitic helminths was 49.1% (270/550), with Ascaris lumbricoides being the most prevalent (30.37%). Multivariate analysis revealed significant associations between intestinal parasitic helminths and lack of hand-washing, insufficient toilet facilities, consumption of unwashed vegetables, and poor environmental sanitation. Males had a significantly higher infection rate than females (P < 0.05). Age-related prevalence was highest among children aged 4-6 (51.85%). Occupational status and educational level of parents were also significant predictors of infection. These findings highlight the need for public health education, provision of basic sanitation facilities, and regular de-worming as preventive measures.

Exploring the Mechanism of Complex Lemon-Angelica Sinensis-Boswellia Essential Oil on Anxiety Disorders with Melasma Through Network Pharmacology and Experimental Validation

The incidence rate of melasma is notably high among patients with anxiety disorders. Aromatherapy primarily influences the physiological and psychological states of individuals through the inhalation or application of essential oils, thereby facilitating the treatment or alleviation of various conditions. This study aims to explore the action mechanism of complex lemon-angelica sinensis -boswellia essential oil (CEO) in treating anxiety disorders with melasma. We investigated the active ingredients, targets, and pathways of CEO in relation to anxiety and melasma using network pharmacology. We employed cell assays and conducted nebulized essential oil inhalation tests on CUMS mice to validate the intervention effects of CEO on anxiety. A total of 28 active components, including neryl acetate, 3-butenylphthalide and octyl acetate, and 26 cross-targets, such as ESR1, CCND1 and PIK3CA, were identified. GO and KEGG pathway analyses indicated that these cross-targets were primarily involved in endocrine regulation, cell proliferation, and apoptosis, specifically through PI3K/Akt signaling pathway and calcium signaling pathway. The experimental results demonstrated that CEO significantly reduced the secretion of NO, TNF-a and IL-6, as well as the mRNA expressions of ESR1, CCND1 and PIK3CA in cells compared to the inflammatory cell model. Furthermore, CEO notably decreased the forced swimming immobility time of mice and the levels of IL-1β, ESR1 and CCND1 in hippocampus when compared to model mice. These findings suggest that CEO may regulate ESR1, CCND1 and PIK3CA through its citral, 3-butylphthalate and neryl acetate, thereby influencing endocrine function, cell proliferation and apoptosis, inhibiting inflammation and anxiety-like behavior in CUMS-induced mice.

Recruitment Strategies and Challenges in a Pilot HIV Prevention Study among Cisgender Black Women in Houston, Texas

Recruitment for research studies focused on communicable diseases such as HIV (Human Immunodeficiency Virus) has historically been challenging, particularly among populations who have been underrepresented in media messaging, yet maintain a significant vulnerability to new HIV cases, like Black women. This study examines the recruitment strategies utilized in Aim 2 of pilot study at the University of Texas Health Science Center (UTHealth) in Houston, Texas that was funded by the Ujima Mentoring Program to develop, implement, and evaluate a video log (vlog)-based intervention. The overarching goal of the pilot study was to promote the uptake of pre-exposure prophylaxis (PrEP) among cisgender Black women in Harris County, Texas. Aim 2 involved the creation of a culturally relevant vlog for use in Aim 3, which sought to educate and motivate participants to consider PrEP as an HIV prevention strategy. With the growing role of digital platforms in public health outreach, social media was employed alongside traditional recruitment methods such as flyers and word-of-mouth referrals. Despite an expanded online reach, engagement remained low, and participation rates did not increase proportionally to the number of individuals who viewed recruitment materials. The reluctance to participate was largely attributed to stigma surrounding HIV and concerns about being publicly associated with an HIV prevention study based on participants feedback to the research team. Even the availability of research incentives to encourage participation did not significantly improve recruitment accrual goals, particularly among healthcare providers who experienced limitations with eligibility due to institutional policies. This study underscores the need to better understand the social and cultural barriers that prevent Black women and healthcare providers from engaging in HIV prevention research. While digital recruitment strategies can enhance visibility to a larger audience, they must be supported with trust-building efforts, community partnerships, and culturally competent messaging to encourage meaningful participation. These findings point to the critical need for multi-faceted recruitment strategies that go beyond social media campaigns and actively build trust within communities, ensuring that HIV prevention research and interventions are both accessible and culturally resonant.

Toward Better Care for Sickle Cell Disease in Nigeria: A Review of Challenges and Interventions

With more than 150,000 affected infants annually, Nigeria bears the largest burden worldwide of sickle cell disease (SCD), making it a significant public health concern. The management of SCD in Nigeria is challenging, despite advancements in medical research and increased knowledge. This review examines the numerous issues surrounding SCD in the nation, including the financial burden on affected families, the lack of specialized care facilities, the absence of newborn screening programs, the sociocultural stigmatization of SCD, and restricted access to high-quality healthcare. Additionally, inadequate public health education and a lack of coordinated national policies result in delayed diagnosis and suboptimal treatment outcomes. We also highlighted recent efforts and recommendations aimed at improving early detection, comprehensive care, and community support. Addressing these challenges through expanded health education and enhanced healthcare infrastructure is essential to reducing morbidity and mortality associated with SCD in Nigeria.

Menopausal Symptoms Affecting Productivity and Occupational Needs of Peri-Menopausal Women in a Private University, Philippines

Objective This study aimed to explore the respondents’ reproductive health profiles, examining the physiological and psychosocial perimenopausal symptoms affecting productivity, and identifying occupational needs. It also determined the relationship between menopausal symptoms and the productivity and occupational needs of peri-menopausal women. Methodology A descriptive cross-sectional design was used, surveying 50 women over 40 years old with a validated four-point Likert scale instrument. The instrument undergone content validation, reliability test, and ethical approval. Survey was administered personally and online using purposive sampling. Statistical treatments included weighted mean, F-test, T-test, Pearson r correlation, and ranking. Key results The majority of participants had their first menstruation between 11 to 15 years old and experienced regular menstrual cycles. Most had one child, with an equal number of cesarean and normal deliveries, and reported no pregnancy complications. The study found that participants seldom experienced physiological and psychosocial menopausal symptoms. They agreed on the occupational needs during the perimenopausal period. It was found that physiological symptoms were influenced by factors such as early menarche, cesarean delivery, and pregnancy complications. Additionally, psychosocial symptoms varied based on menstrual status, the number of children, and pregnancy complications, with those experiencing earlier menstruation or complications reporting more intense symptoms. The study revealed a significant relationship between both physiological and psychosocial perimenopausal symptoms, which negatively impacted productivity and increased occupational needs. Women with higher menopausal symptoms expressed a greater need for workplace policies that support perimenopausal women, highlighting the need for tailored workplace interventions for this demographic. Future Direction The study recommends including pap smears and mammograms in annual exams for peri-menopausal women, offering awareness seminars on managing perimenopausal symptoms to reduce workplace disruptions, and suggests future research exploring additional variables affecting perimenopausal women’s health and productivity

Retrospective Evaluation of Well Differentiated Thyroid Cancer Treatment Outcomes: 50 year experience at the University of Puerto Rico

Our study gathered information on the diagnosis, treatment, and long-term outcome in adult and pediatric Hispanic patients with Well Differentiated Thyroid Cancer. Methods We performed a retrospective review of the clinical and imaging nuclear medicine records of cases of WDTC evaluated and treated in the Nuclear Medicine CLINICc. Evaluation included the clinical PROFILE, histology, radioiodine (RAI) therapies and treatment response, long-term outcome and survival. The data was ASSESED using the 2015 ATA Risk level guidelines and recommendations. Results Three hundred eleven cases were reviewed, 81% females, 19% males, median age of 41 years. Eleven percent (34 patients) of the patients were in the pediatric group and 49% were between 16-45 years. The tumor histology was 60.5% Papillary, 28.2% Papillary-Follicular variant and 11.3% Follicular type. All patients had a total thyroidectomy. A total of 287 (92%) of the patients were treated with RAI. The median RAI dose was 128 mCi. Patients in the low risk group received a dose range of 25-105 mCi, 73 cases in the intermediate RISK group received 106-160mCi and 104 cases in the high-risk group received doses greater than 160 mCi. The overall median cumulative dose was 151 mCi (55-926 mCi). Annual follow up was done in all cases , WITH A median follow-up OF 5-10 years. Residual functioning tissue in the neck was found in 52% of the cases by US and/or RAI imaging. of those, 43% belonged to the low risk group, while 57% were in the intermediate and high-risk groupS. The mean treatment dose received by those with persistent functional thyroid tissue in the neck was 157 mCi. Recurrent disease was found in 15% of the patients, 85% of them belonged to the intermediate and high-risk GROUPS. Forty-seven percent of the patients with recurrent disease had residual disease. Conclusion We believe ablative and/or adjuvant RAI treatment early in the disease is important to decrease residual thyroid tissue and/or residual disease, and to improve disease-free survival. We recommend total thyroid surgery in all tumors above 1 cm, post-operative evaluation with RAI Whole Body (with 123-I or 131-I), planar and SPECT/CT imaging and RAI ablation to remnant tissue. Follow-up post treatment evaluation is also recommended.

Efficacy of Neemraj and Beauvitech on Abundance and Distribution of Arthropod Pests in Soil and Hydroponically Grown Strawberry

Strawberry is a commercially important crop which has gained global significance in terms of production and consumption. The strawberry farmers have been faced with economic losses due to attack from arthropod and phytophagus pests. To overcome this challenge, management of pests is important with reduced impact on beneficial organisms and the environment. Use of non-chemical approaches as an alternative to insecticide driven management is highly gaining popularity. Identification of the non-chemical production for management of strawberry pest such as entomopathogenic fungi and botanicals is critical. The need for compatibility of entomopathogenic fungi in crop production techniques with other insecticides to enhance efficacy is required. The interaction between these control agents could be synergistic, additive or even antagonistic. Synergistic interactions usually lead to enhanced effectiveness of the microbial control agents while reducing the adverse effects of the pesticides in combination. In this review a description of the compatibility of entomopathogenic fungus Beauvaria bassiana and botanical neem in strawberry pest management is given. The experiment involved five treatments: Neemraj, Beauvitech, Neemraj and Beauvitech, Ampligo and control. These treatments were allocated in a completely Randomized design in two separate greenhouse setups one with soil and other with pumice. The treatments were replicated four times. The data was subjected to analysis of variance (ANOVA) using the general linear model (GLM) procedure of the statistical analysis system (SAS) program, version 9.1. Tukey’s (HSD) test at P≤ 0.05 was used to separate significant means. The combination of Neemraj and Beauvitech was most effective in the management of pests in both hydroponics and soil grown media. Pest populations were reduced significantly as follows; thrips, Frankliniella spp 2.1, red spider mites Tetranychus species 11.5, tarnished plant bug Lygus species 3.0, there was 0% cricket Gryllus species damage and 0 number strawberry weevil in hydroponics cultures while in soil culture pest mean populations were: thrips 5.6, spider mites Tetranychus species 18.2, strawberry weevil (Otiorhynchus species) 7.0, tarnished plant bug 3.0 and there was 13.8% cricket Gryllus species damage on strawberry plants.

The growing population demands and environmental concerns associated with traditional protein sources have prompted the exploration of alternative and sustainable food sources. The purpose of this comprehensive review is to highlight the nutritional benefits and sustainability of insect-based foods as a promising solution. Global population growth necessitates innovative approaches to meet the demand for nutritious and sustainable protein sources. There are numerous challenges associated with traditional livestock farming, including land use inefficiency, high water usage, and greenhouse gas emissions. As a result, edible insects have emerged as a viable alternative, providing proteins (35-77% of dry matter), healthy fats (10-50%), essential amino acids, and micronutrients such as iron (up to 31mg/100g) and zinc (up to 20mg/100g), vitamins, and minerals. In contrast to livestock, which requires 22,000-43,000 liters of water to produce 1 kg of beef, insect farming consumes significantly less water and land resources. Insects have the potential to address nutritional deficiencies and strengthen food security as they are recognized for sustainable production. The study thoroughly investigates the literature addressing environmental and sustainability concerns associated with edible insect farming, using a rigorous bibliometric and scientometric analysis via Vos viewer. With the help of Vos Viewer, it was possible to identify the geographical distribution of countries that contributed to the field of edible insects and their acceptance, as well as the top ten documents in this field with the most citations and mostly used keywords in this field of research. Future research and implementation strategies will be able to benefit global food security and environmental conservation through these alternative protein sources.

HIV-Associated Peripheral Neuropathy and Antiretroviral Therapy: A Prospective Study from a Tertiary Care Centre in South India

Background Peripheral neuropathy (PN) is a common and debilitating complication in people living with HIV (PLHIV). While HIV itself contributes to neuropathy, certain antiretroviral therapy (ART) drugs, particularly nucleoside reverse transcriptase inhibitors (NRTIs) such as stavudine (d4T) and zidovudine (AZT), are known for their neurotoxic effects. Objectives To evaluate the impact of ART on HIV-associated peripheral neuropathy (HIV-PN) and to determine whether certain ART regimens increase the risk or severity of neuropathy. Materials and Methods A cross-sectional study was conducted among 158 HIV-positive patients. Neuropathy was diagnosed using clinical criteria, Total Neuropathy Score (TNS), and nerve conduction studies (NCS). Patients were grouped based on their ART regimen, and statistical analysis was performed to assess the association between ART type and peripheral neuropathy severity. Results It was noted that patients on older NRTIs (stavudine, zidovudine) had significantly higher rates of peripheral neuropathy (p=0.002) and tenofovir-based regimens were associated with lower peripheral neuropathy prevalence (p=0.01). There was a significant correlation between the duration of ART exposure and peripheral neuropathy severity (p<0.001), suggesting a cumulative neurotoxic effect. Conclusion Older ART regimens, particularly stavudine and zidovudine, significantly contribute to HIV-PN. The study supports the WHO recommendation to phase out neurotoxic ART and highlights the importance of early ART regimen optimisation to prevent long-term neurological complications.

Perceived Physical Progress and Mental Improvement Among Midlife Women Practicing the 3D Movement Method: A Brief Report from a Cross-Sectional Survey

Background Mind–body exercises are frequently used to support well-being during the menopausal transition, but perceived physical and psychological outcomes associated with practices such as the 3D Movement Method have not been quantitatively assessed. Objective To examine associations between characteristics of the 3D Movement Method and women’s self-reported physical and mental improvement during the menopausal transition. Methods Cross-sectional survey data from 330 women aged ≥ 35 years were analyzed using general linear models. Perceived physical progress and mental improvement were each assessed using single-item, 15-point scales. Independent variables included practice frequency, duration of experience, explanation clarity, satisfaction, baseline health status (SF-36 Physical and Mental Component Summary scores), age, and menopausal status. Results The model for physical progress was significant, F(16, 144) = 6.26, p < .001, R² = .41. Greater practice frequency, longer experience, clearer instructional explanations, and higher satisfaction were each independently associated with higher perceived physical progress. The mental improvement model was also significant, F(16, 96) = 2.25, p = .008, R² = .27, identifying satisfaction as the sole significant predictor. Baseline health, age, and menopausal status did not predict outcomes in either model. Conclusions Perceived physical progress was shaped by structured engagement parameters consistent with deliberate-practice principles, whereas perceived mental improvement was primarily linked to subjective satisfaction, a pattern compatible with motivational accounts of engagement. The 3D Movement Method may support women’s health across the menopausal transition when designed to optimize clarity, engagement, and experiential quality.

Addressing an Overlooked Population: The Role of Discrimination and Violence in Depression Among South Asian Female College Students

South Asian female college students in the United States face mental health challenges shaped by intersecting experiences of discrimination and violence. This study examined how discrimination and violence contribute to depression in this population. An anonymous, cross-sectional, web-based survey (N=673) was distributed nationally through South Asian organizations, listservs, and social media. Validated measures assessed day-to-day discrimination, college-based discrimination, experiences of violence during college, and depression (PHQ-9). Bivariate analyses and multivariate linear regression examined associations, adjusting for sociodemographic factors. Over half of participants (51.1%) reported college-based discrimination, 66.1% reported experiencing violence during college, and 25.7% met the criteria for depression. In adjusted models, day-to-day discrimination (β=0.261, p<0.001) and college violence (β=0.207, p<0.001) were significant predictors of depression. Bisexual and questioning/unsure students also reported higher depression scores than heterosexual peers. Discrimination and violence are key social determinants of mental health among this population. Findings underscore the need for culturally responsive mental health services, intersectional campus policies, and evidence-based interventions to promote health equity among minority women in higher education.

Efficacy and Safety of Lercanidipine Combination in Hypertensive Patients

Calcium channel blockers (CCBs) are widely used for the treatment of arterial hypertension, but they differ in terms of pharmacology, tolerability, and pleiotropic actions. Lercanidipine, a highly lipophilic third generation dihydropyridine, reduces blood pressure (BP) effectively as monotherapy and in combination without inferiority to other major antihypertensive classes. We systematically searched PubMed and the Cochrane Library (last update: September 1, 2025) and screened reference lists for additional studies. Evidence from dose finding trials, randomized controlled studies, large observational cohorts, and meta analyses shows clinically meaningful reductions in office, home, and ambulatory BP with lercanidipine, including in patients with diabetes, obesity, chronic kidney disease, or high cardiovascular (CV) risk. Fixed- dose combinations with renin angiotensin system blockers (e.g., enalapril) provide greater BP reductions than monotherapy and are associated with favorable neurometabolic profiles. Beyond BP control, lercanidipine improves central hemodynamics and arterial stiffness, favors endothelial biology, and contributes to left ventricular hypertrophy regression. Across comparative trials, lercanidipine is generally better tolerated than older dihydropyridines. Presents lower rates of vasodilatory adverse events, less sympathetic activation, while discontinuations due to adverse events are uncommon. Overall, lercanidipine particularly within single pill combinations offers effective, durable BP lowering across diverse patient profiles with a favorable safety and tolerability profile and pleiotropic benefits that extend beyond BP reduction. Assessing The Nutritional Status and Health Outcomes of Women and Children in Rajshahi, Bangladesh: A Comprehensive Study

Background Malnutrition is a significant public health issue in Bangladesh, particularly impacting women and children. Rajshahi, marked by socio-economic disparities, offers a distinctive context to explore the nutritional status and health outcomes of these vulnerable groups. Objectives This study aims to assess the nutritional status of women and children in Rajshahi and investigate associated health outcomes. Additionally, it seeks to identify socio-economic and cultural factors that influence nutrition. Methods A mixed-methods approach was utilized, incorporating a cross-sectional survey of 460 households and in-depth interviews with mothers and caregivers. Anthropometric measurements were taken to evaluate the nutritional status of women and children, while dietary assessments measured nutrient intake and diversity. Logistic regression analysis was performed to determine the likelihood of malnutrition based on socio-economic characteristics, thereby identifying key risk factors. Results The findings indicate a troubling prevalence of malnutrition, with 36% of children under five classified as stunted and 25% as underweight. The analysis highlights critical factors contributing to chronic undernutrition, including maternal education, employment, and dietary diversity. Notably, mothers aged 27-37 exhibit a lower risk of undernutrition, and urban households with secure food access demonstrate better nutritional outcomes. Discussion Maternal education and employment were positively associated with better nutritional outcomes, as educated and employed mothers had higher chances of maintaining a normal BMI. Regular ANC visits (≥4 visits) were crucial for improved maternal nutrition. Household food security emerged as a significant determinant, with food-secure households showing better maternal nutritional status. Safe water access and adequate dietary diversity were also linked to improved maternal BMI. Additionally, factors such as child birth weight, exclusive breastfeeding, and childhood diarrhea significantly influenced maternal nutrition. Moreover, frequent antenatal care visits and a diverse diet are vital in mitigating undernutrition risks among children. Conclusion This study emphasizes the urgent need for targeted interventions to combat malnutrition in Rajshahi. Recommendations include implementing community-based nutrition education programs and improving access to healthcare services. By addressing the socio-economic and cultural determinants of nutrition, stakeholders can enhance health outcomes for women and children in the region, ultimately contributing to broader public health objectives in Bangladesh.

Caregiver-Child Co-Rumination and Treatment Outcomes in a Randomized Clinical Trial of Rumination-Focused Cognitive-Behavioral Therapy

A number of developmental factors increase risk for adolescent rumination. This particular kind of repetitive negative thinking pattern often begins in the context of familial stressors and parental modeling. Though rumination can be effectively targeted with rumination-focused cognitive behavioral therapy (RF-CBT), it is unknown whether caregiver-child co-rumination (1) affects caregiver views of their child’s psychopathology, (2) or interferes with youth rumination- focused treatment. The present study uses data from a randomized clinical trial of RF-CBT to examine whether caregiver-child co-rumination, or caregivers’ own rumination patterns, are associated with bias in parental perception of their adolescent’s depression symptoms. We also examine if co-rumination scores at baseline moderate rumination scores for youth at treatment termination, and whether treatment effects dampen or decay more significantly post-treatment among youth with higher caregiver-child co-rumination. Youth (N = 76) were randomized to either 10-14 sessions of RF-CBT (n = 38) or treatment as usual (TAU; n = 38) and completed interviews and surveys at pre-treatment baseline, post-treatment, and 3-, 6-, 9-, and 12-month follow up. Results indicate that neither caregiver rumination nor co-rumination scores bias caregivers’ views of their child’s depression symptoms. In terms of reduction in child’s rumination scores, estimated treatment effects were larger for patients with higher baseline co- rumination scores, and there was no statistically significant difference in treatment effect decay over time in high versus low co-rumination groups. Results indicate co-rumination overall does not dampen the effect of RF-CBT, and those experiencing the highest levels of co-rumination may benefit most from treatment.

Social work has its roots in the efforts of early twentieth-century charity organizations to reduce human suffering. This movement among charity workers of the era was founded in the search for theoretical knowledge that would illuminate effective treatment options for social ills and ultimately transform the provision of charity into social work. Coinciding with the birth of social work was the development of psychoanalysis (PA) as a tool for theorizing and treating mental illness. For many decades training in PA theory was common within social work graduate programs and many clinical social workers practice today psychotherapy informed by PA principles. However, clinical social workers were long excluded from enrolling in and graduating from psychoanalytic training institutes, a requirement for being able to say that one is a psychoanalyst and provides psychoanalysis. In 1985, a legal ruling asserted that psychologists and other qualified mental health professionals could not be excluded from enrollment in PA training programs. Since that watershed decision, although many social workers have gone on to enroll and graduate as psychoanalysts, the process whereby one can pursue this training path is unfamiliar to most social workers. We describe the current PA training landscape and describe the process on how a LCSW can become a legitimate psychoanalyst.

Advancements in proteomic and genomic technologies have transformed molecular biology by enabling comprehensive analysis of biological systems at the molecular level. This literature review explores the evolution, methodologies, and practical applications of key proteomic and genomic techniques. In proteomics, tools such as two-dimensional electrophoresis, mass spectrometry, Western blotting, Edman degradation, and functional protein microarrays have facilitated high-throughput protein identification, post-translational modification analysis, and biomarker discovery. Similarly, genomic methodologies like PCR, recombinant DNA technology, gel electrophoresis, and Southern blotting have revolutionized gene detection, manipulation, and expression profiling. The review also highlights the interdisciplinary impact of these technologies across clinical diagnostics, oncology, autoimmune disorders, infectious disease surveillance, cardiovascular research, and personalized nutrition. Integrative approaches combining proteomics and genomics are enabling the discovery of novel therapeutic targets, improving disease classification, and advancing precision medicine. Despite current limitations, such as the absence of amplification techniques for proteins and challenges in data interpretation, ongoing innovations promise to bridge these gaps. This synthesis underscores the pivotal role of molecular techniques in deepening our understanding of human biology and accelerating biomedical advancements for improved healthcare outcomes.

Review on Frequency Neurofeedback on Autism Spectrum Disorder: Overview, Efficacy and Research Direction

Frequency neurofeedback (FNF) is a biofeedback method that targets frequencies between 1 and 50 Hz. The efficacy of FNF with autism has been labeled ‘probably efficacious’ in literature reviews in the last decade, despite new research pointing towards a higher standard. The aim of this review was to analyze key features of these studies, with a goal of determining the efficacy standard of FNF on autism and establishing a research direction. Electronic databases and literature reviews were used to collect a total of ten randomized and/or matched controlled trials. FNF reaches a Level 4 efficacy standard, with an impact on a broad range of factors including core autistic traits, social communication, emotional regulation, cognitive flexibility, executive function, behaviors of concern, attention, metabolic or thermal activity, and EEG e.g. decreased absolute power, mu rhythm, coherence and hyperconnectivity. Current evidence generalizes to male children, up to 18 years, with a low-average or higher intellectual functioning, with autism as the only diagnosis. A meta-analysis suggests a large superior effect when compared to wait list controls. Current research does not meet the higher efficacy standards outlined by Arns et al. (2020). Small samples plague most studies, and the maintenance of improvements post-training are yet to be assessed adequately. Eight recommendations are made.

Systematic Review of Spinal Cord Injuries in Equestrian Athletes: Incidence, Risk Factors, and Outcomes

Objective The goal of this systematic review is to identify common themes amongst acute spinal cord injuries (SCI) in equestrian athletes. Design A systematic review was performed using PubMed, CINAHL Plus with Full Text (EBSCO), Cochrane Library, and Scopus with pre-determined MESH terms. The initial search returned 354 studies. Following PRISMA guidelines, 13 articles were included. Exclusion criteria included injuries to the horse only, non-English language, cauda equina, and case reports. Data extraction was completed, and common findings were evaluated narratively due to heterogeneity of data. Results Seven manuscripts listed specific horse-related activities that caused SCI, with fall from horse as the highest percentage of injury. Nine articles identified the injury region, with large variations and no clear dominant area of injury. Five articles identified the length of hospital stay with ranges from 1 to 82 days. Four articles looked at the association of professional vs non-professional riders. Only two articles evaluated helmet use at time of injury, with one article showing 81% of those with SCI used helmets, and the other showing only 35.6% utilized this safety measure. Conclusion SCI in equestrian athletes can have a wide presentation, with large variation on location of injury, length of stay, and other factors. However, non-professional riders are at greater risk of SCI and individuals are more likely to sustain injury from a fall from a horse rather than a kick or another modality of injury. Future study can elicit presenting symptoms, types of surgical intervention used, and long-term outcomes and recovery.

Traumatic Maculopathy (Berlin´S Edema) Treated with Enhancement of Human Photosynthesis®

Commotio retinae (CR) is a condition frequently observed in clinical practice, particularly following closed globe trauma (CGT) due to sport, labor, or traffic accident injuries. It is the main cause of unilateral vision loss in male patients aged between. It is characterized by transient gray-white retinal coloration and reduction of visual acuity (VA). Symptoms depend mainly on the location and severity (deep) of the injury, with less complains when only the superficial or peripheral retina is affected. It may be confined to the posterior pole, when it is also called Berlin’s edema, after the first hypothesis of Berlin (1873). There is no specific treatment since the treatment depends on the region of the retina and choroid affected. In this work we report a case of Berlin edema, treated with ǪIAPI 1®, to restore the balance of oxygen, which is generated at the intracellular level.

Ayurveda is a traditional and scholarly medical system practiced in South Asia since the Vedic period. It integrates science and philosophy to promote a healthy, happy and prosperous life. Recent studies have highlighted that bhasmas possess antioxidant, anti-inflammatory, antibacterial, antiviral and antitumor properties and may serve as potential carriers for drug delivery. Prevalence of Hypertension Among Adolescents in Benue South, Nigeria

Background Hypertension in adolescents has been shown to tracks into adulthood, as well as causing premature cardiovascular and renal diseases. Objectives To determine the prevalence of elevated blood pressure and hypertension among adolescents of ages 10-19 years old attending secondary schools in Benue South, Nigeria; characterise their demographics and determine factors associated with the development of high blood pressure. Materials and Methods A cross-sectional descriptive study of secondary school adolescents selected through multistage sampling from across three Local Government Areas of Benue South, Nigeria. All the participants had their blood pressure measured using mercury sphygmomanometer and their height and weight taken for the calculation of their body mass index. A dipstick urinalysis with was carried out on their urine samples. Data analysis was with SPSS version 25. Results Atotal of 260 adolescents were studied, males were 132 (50.8%) and the mean age was 13.65 ±2.01 years. The prevalence of elevated blood pressure and hypertension was 5.4% and 2.3% respectively. Fifteen females (75.0%) had elevated blood pressure/hypertension as against five males (25%) and it was statistically significant. Adolescents in mid-adolescence age (60.0%) and lower social class (70.0%) had higher rate of high blood pressure. Significant proteinuria (+) was found among eleven (55.0%) of those with high blood pressure. Conclusion The prevalence of elevated blood pressure and hypertension among the adolescents was 5.4% and 2.3% respectively; being female, within mid-adolescence age and from lower social class are associated factors. Recommendation Regular blood pressure measurement should be part of school health programme.

Learning and Memory in an Animal Model of Longevity: The Ames Dwarf Mice

The Ames dwarf mice have a recessive mutation of the PROP-1 gene that produces hereditary dwarfism. The abnormality is responsible for an anterior-pituitary deficiency that results in a substantial reduction of growth hormone, thyroid-stimulating hormone, and prolactin. These mice are smaller in size than their normal siblings but live approximately twice as long. The normal siblings do not have the mutation, and therefore still have the typical levels of the three hormones. The purpose of the present research was to determine if the reduced hormones in the Ames dwarf mice affected their ability to learn and delayed the age-related loss of memory. In general, the hypotheses proposed indicate that there will be no significant differences on the tasks in regards to the genotype or the age of the mice. These hypotheses would support previous research and suggest a delay in the age-related loss of memory and the ability to learn in the Ames dwarf mice. Learning was assessed using a matching-to-sample procedure, while memory was evaluated using a modified radial-arm procedure. Generally, the age of the animals had little to do with their performance on any of the tasks. Taken together, the overall results showed no significant differences in accuracy between any of the groups of mice or a behavioral decline as the mice age. The present results are consistent with the theory of a delayed age-related behavioral decline in the Ames dwarf mice.

It is proposed that the human soul is a manifestation of a soul field consisting of a collection of quantum-like fields. The soul field interacts with the electromagnetic field, manifested by photons interacting with the quanta of the soul field. Evidence for this comes from near-death experiences where reported events that could not have been seen through the eyes of the individual are verified. Since bioelectric fields are a type of electromagnetic field, bioelectric fields may also interact with the soul field. This could result in the transfer of information on working memory content to the soul via interactions with bioelectric fields produced by neural ensembles in the human brain. The soul field may also affect neurons on the molecular level in the brain through interactions with bioelectric fields and the recently proposed mechanism of cytoelectric coupling. The human soul is coupled to the body through its interactions with bioelectric fields in the body. Manifestations of the quantum-like fields comprising the soul field may carry out different functions such as encoding memories and experiences, representing emotion states, and defining personal identity. Interactions of these fields and their quanta could produce emergent properties such as self-awareness and consciousness.

This review summarizes current and future applications of AI in healthcare, focusing on its potential to improve diagnosis, therapy, chronic disease management, and overall patient care, while also alleviating physicians’ workload. Recent literature demonstrates that AI systems can reduce diagnostic errors/delays by mitigating cognitive biases, support imaging and pathology through improved accuracy and speed, and prevent prescribing errors by integrating pharmacogenomic and clinical data into decision-support systems. In chronic disease management, AI-powered wearable devices enable continuous monitoring and early detection of conditions such as atrial fibrillation, thereby reducing the risk of stroke and long-term disability, particularly in elderly people. Therapeutic applications include AI-driven drug discovery, personalized oncology, and tailored medicine that integrates multi-omics and lifestyle data. Beyond direct medical intervention, AI contributes by automating routine tasks, optimizing workflows, and facilitating greater patient–clinician interaction. Despite these benefits, significant challenges remain, including issues of data quality, privacy, security, equity, and the need for transparency and trust in “black box” systems. Looking ahead, the integration of multimodal data, digital twins, and robotics is expected to advance more comprehensive, equitable, and human-centered care. We conclude that, when applied ethically and responsibly, AI should not replace clinicians but rather serve as a powerful partner that enhances medicine by restoring empathy and humanity.

The Brainstem Auditory Evoked Potential: A Reinterpretation of its Electrogenesis

Depending upon the species, the brainstem auditory evoked potential (BAEP) consists of four or five major high frequency components. According to longstanding doctrine, each wave represents the sequential activation of successively higher nuclei and tracts from the 8th (auditory) nerve to the midbrain (inferior colliculus). Although this conceptual framework has acquired the status of near dogma, surprisingly little evidence exists in support. In the present analysis, a new interpretation of the electrogenesis of the BAEP is proposed which is simpler although it retains skeletal elements of the older explanation. The revised model is mostly derived from two distinct sources. In the first, the timing of the BAEP waves is compared with that of cortical activity for a range of mammals including humans, monkeys, cats, rats and guinea pigs. It is demonstrated that for each of these, the conduction time of the acoustic signal to the cortex from the putative midbrain component (wave IV or V) is so unrealistically long that it implies that the entire waveform must arise in the peripheral pathways of the auditory system. In the second, a retrospective analysis is made of click repetition rates on the BAEP using extradural electrodes. It was shown that at high rates of stimulation (about 100/sec), the behavior of the waveform is almost totally at variance with the expectations of the conventional model. The essence of the revised conception is that all BAEP waves are just variations of the compound action potential of the 8th nerve, albeit generated or regenerated via separate routes and different methods. Such an explanation would thereby account for their near uniform sharp morphology as well as creating the impression of a composite neuronal response. More specifically, in the case of a four component BAEP, wave I is assumed to be generated by the normal air conduction route in an identical manner to the conventional explanation. In contrast, wave ll is assumed to be generated via bone conduction in the temporal skull thereby bypassing the transduction process in the middle ear. Wave lll is assumed to be generated by the first echo of the bone-conducted sound wave. Likewise, the second rebound within the temporal bone serves as the stimulation to evoke wave lV. As the energy of the auditory stimulus gradually dissipates, it may still continue to generate a train of lower amplitude potentials. It is concluded that the BAEP may contain little or no brainstem or midbrain activity and therefore the term BAEP may be a misnomer. A more appropriate epithet might therefore be the auditory nerve evoked potential or ANEP.
